Item 6 - Self explanatory. <br />Item 7 - Please list whether this is an explorntory, active, oruractive site, Ifthe site is in temporary cessation orpost-mining, please sffite. The Division defines <br />[eons as follows: <br />Esploratory -An exploratory mine is one in which analysis is being done to detemrine if active mining is economical. Any active mining oceum <br />to 1972 or the site was previously rechsimed and released from bond by the Colorado Division of Mmemis and Geology (CDMG). <br />Active - An active mine is ono in which coal is being mined or has been mined since 1972. A faciliTy is considered active by the Division until th~ <br />CDMG releases the facility from bond. <br />Temporary Cessation -Temporary cessation is a CDMG taint which addresses those mines which are not actively producing coal but which ha• <br />commenced final reclamation or closure of the site. The Division considers such facilities as active. <br />Post-Mlning -Amine or an area of a mine is considered to be in post-mining when the surface area of a coat mine has barn returned to the reyui <br />contours and which vegetation (reseeding) work has commenced..4n area is considered to be in post-mining until the bond has been released fro <br />CDMG. <br />If a mint is exploratory, it is requested that information on the quality of the stream and any discharges be submitted. Failure to do so may result in difl <br />later should you request inactivation. <br />Item 8,9 - Self explanatory. <br />Item 10 - A discharge is intermittent unless it occurs without interruption during the operating hours of the facility, except for maintenance, process change or sv <br />shutdown. A discharge is seasonal if it oceurs only durng certain parts of the year. <br />Item 11 - This map is inirnded to serve as en area map for inclusion yr the permit. A legible 82 : Il me6 sabnrittal is required. A larger map may be include <br />reference if the pernittce wishes. If maps are no[ supplied, issmance of vonr Hermit will be delayed. <br />Item 12 - This is a facility sketch for inclusion in the pernit A legible 82 z 11 inch submittal v required. If sketch is not supplied, or if it is larger than 8 2 x <br />inches, issmanm of your cermit w8I be delayed, <br />Item 13 - The line drawing should show generally the mute taken by water in your facility tram intake to discharge. Show eU operations conhibuting wastewater <br />including process and production areas. The water balance should show average flows and all significant losses of water to products, the atmosphere, < <br />cemrol and discharge. Use actual measurement whenever possible, otherwise use your best estimate. If a water balance is not appropriate, please iodic <br />Item 14 - Mark which of the other permits/progtatns the 1-scility has obtained or is in the process of obtaining or is subject to regulation under. <br />See items 37 and 38 regarding Stormwater requuemrnffi. <br />Item 15 - Self expbvratory. <br />Item 16 - List here those chemicals or materials added to the water prior tq during, and otter the water is used and prior to discharge. Also fist chemicals or mater <br />used th the treatment of the wastewaters. <br />Item 17 - If applicable, list those toxic products that are used N the manufacturing process or are produced by the manufacturing process. Do not include those <br />substances listed in item 16. <br />Item 18 - Self explanatory. <br />Item 19 - List any improvemrnffi to the wastewater system which are required or which are being underffikrn to rnsure future compliance with rnvironmrntal <br />regulations. The Division may request additional information concerning these changes during the application review process. <br />Item 20 - If land application, deleted as any discharge being applied to the land for treatment purposes, is practiced or proposed the Division needs appropriate <br />information to understand the operation and make ajudgemrn[ as to possible impact on surface waters. Tf not identified elsewhere, idrntify the nearest <br />surface waters or dry stream bed. Land application of water: is subject to the Colorado Division of Minerals and Geology review and approval. Please <br />include the infomration submitted to CDMG to obtain such approval and a copy of the letter rceeived granting such approval. <br />Item 21 - List the outfall number for each discharge point. List all sources of wastewater for each ou[fafl and give the 30 day average flow and daily matdmum <br />flow. You may estimate the flow contributed by each source if no daffi is available, and for stornwater, you may use any reasonable measure of <br />dnrstimn, volume or 15equrncy. Describe each treatrnrnt unit Indicate the 10-year, 24-hour equivalent volume used in designing the treatimem system <br />and the system's actual volume, excluding solids retention and any "permanrnt pool" that may be provided. Indicate if exha capacity is provided for <br />mine water and/or other von-storm related flows and how this volume was determined. Indicate what type(s) of discharge shuclure each outfall has <br />and how flow iadischarged - whether it discharges automatically or manually. If your flows vary significantly or if you anticipate significant changes in <br />flows during the nett 5 years, specify which flows will change and explain why they will change. Describe the ultimate disposal of any solid or liquid <br />waste not discharged. Use additional pages as necessary. <br />Item 22 - List the outfall, latitude and longitude for each outfall location and the receiving waters. This information is vital for the Division=s new information <br />data system. Please indicate whether latitude and longitude were detemtined through use of a Giobal Positioning System device, through mapping <br />rmrdinetes or an estimate or other potrntia8y less accurate method. <br />
