-INSTRUCTIONS- <br />WATER QUALITY CONTROL DIVISION <br />COLORADO DISCHARGE PERMIT SYSTEM <br />PROCESS WATER and/or STORMWATER <br />COAL MINING DISCHARGE APPLICATION <br />' T7tis form may be reproduced. For information on electronic copies, please contact the Permits Unit at (303) 692-3500. <br />This application is for use by all coal mining facilities with process water and/or stormwater discharges. It is applicable fm both general and individual pemrit cove <br />The Division has other industry-specific permits including: sand & gravel mining, construction dewatering, gasoline clean up sites, water tmahnent plants, hardmck <br />mining/milling, placer mining and minimal discharge. If the facility falls under one of these activities, please contact the Division for the appropriate application inste <br />this application. <br />' WATER RIGHTS <br />The State Engineem Office (SEO) has indicated that any discharge that does not return water dvecUy to surface waters (i.e. land application, repid infiltration basins, <br />has the potential for material injury to a water right. As a result, the SEO needs to determine that material injury to a water right will not occur from such activities. ' <br />' make this judgement, the SEO requests that a copy of all documentation demonstrating that the requirements of Colorado water law have been met, be submitted to <br />office far review. It is important to understand the <br />' CDPS permit issued by the Division does not constitute a water right issuance of s CDNS permit does not negate Me need to also have the necessary water <br />m place. It is also important to understand that even if the activity has an existing CDPS permit, there is no guarantee Urat the proper water rights are in place. <br />GENERAL INSTRUCTIONS <br />Application Due Dates: At least one hundred sod eighty (180) days prim to the anticipated date of discharge for new facilities m the ezpuation date of an existir <br />permit, the owner (m operator if the owner does not operate the facility) of the facility shall submit an application as provided by ffie Water Quality Control Division <br />"Division"). <br />' Permit Fee: Annual permit fees set by state statute and are collected by the Division. Do not send env oavment with this apmlication. You will be billed once you m <br />covered under a permit Effective Jury 1,1998, there is also a fee for pmtnit amendments. <br />Application Completeness: All items of the application must be completed aceurately and in then entirety or the application will be deemed incomplete, and proses <br />the permit will not begin until all infomration is received. The discussion should extend to the ultimate parent company of each company G: <br />' Item 5 - The Standard Industrial Classification (SIGH Code fm the facility must be listed. SIC Codes are assigoed according to the primary activities performed I <br />company. They are often assigned fm insurance purposes or when a busutess registers as a cotpomtion. Industries can also determine thew SIC Codes 1 <br />checking with thew trade assceiatioq Chamber of Commerce, legal counsel, or local library fm the SIC Ivfmual. <br />
