Item 23 - If the facility generates domestic wastewater from stoke, toiled and/or showers, please complete this item. Indicate the source(s) of such wastewater, <br />what treatment is provided and what method of disposal is utilized (surface discharge, Icach field, etc.). Also, if sludge or other solid waste is generated <br />fiom these activities - describe the source(s), how often i[ is disposed o~ the ultimate disposal site and any other pertinent information. <br />Items 24, 25, 27 - Selfexplanatory. <br />Item 26, 28 These items require monitoring for various pollutand. With the excepfion of item 26 all analyses should be done only at <br />and 29 - outfalls that are not solely made up of storrnwater and domestic wastewater. In item 26, analysis for the indicated parameters shall be performed on <br />each outfafl and receiving waters immediately upstream of the outfafl. If more than one oulfali is to a common body of water, only one analy~s of the <br />receiving water upstream of the upper-most outfall will be required. If the receiving stream is dry during portions of the year, so indicate. In the case of <br />sedimentation ponds for smmrwater runoff, one ou[fall can be sampled if it can reasonably be assumed to be represenhstive of all sedimrndtion pond <br />outfalls. <br />For item 28, the WET testing shalt be conducted on 100% e&]urnt and be for both Ceriodaohnia dubia and fathead mivvows. This regrmement is <br />waived where routine testing is currently required. The test shall be an acute test unless the ratio of stream low flow to effluent design flow is less than <br />10:1, respectively, and the receiving stream has a Class 1 Aquatic Life Use or Class 2 Aquatic Life Use will all the appropriate aquatic life numeric <br />standards. In the latter case a chronic test is required. The Division reserves the right to request additional WET testing es part of the application <br />review. If so requved, the permit will not be processed until the additional information is submitted. WET testing procedures are described in the <br />Guidelines for Conducting Whole Effluent Totdcity Tesd. which can be obtained from the Division. Fot item 29, a priority poiludrrt scan is required <br />unless the probable total annual production is less than 100,000 tons pa year. The paameters for the appropriate GC/MS fraction(s) are shown in <br />Appendix A. <br />For all three items, effluent samples and all insfream samples can be grab samples. Include the sampling date and the name of the laboratory <br />perfomting the analyses. Whrn quantitative data for a pollutant are required, the applicant must collect a sample of effiurnt and analyze it for the <br />poluant m accordance with analytical methods approved under 40 CFR Part 136. When no amalytical method is approved the applicant may use any <br />suitable method but must provide a dcscriptiw of the method. The Division may allow or esdbhsh appropriate sito-spceific sampling procedures or <br />requvemrnd, including sampling locations, the season in which the sampling takes place, protocols for collecting samples and additional time for <br />submitting data on a caso-by-case basis. <br />1T IS RECOMMENDED THAT YOU CONTACT AN ANALYT[CAL LABORATORY PRIOR TO SAMPLING AND ANALYSIS <br />SO THAT PROPER PROCEDURES ARE FOLLOWED. <br />If this is an application for renewal, reduced sampling maybe appropriate, wntact the Pertnid Unit at (303) 692-3500. The Division nmy waive the reporting requvemrnd for individual point sources if the applicant has demonstrated that such a waives <br />is appropriate because information adequate to support iasuertce of a permit can be obtained with less stringent requiremend. Each applicant must <br />report quantitative data for each outfall containing process wastewater with the following exceptions: <br />a) For every polludrtt discharged which is not so limited m an effluent lindmdons guidelinq the applicant must either report quantiative dam or <br />briefly describe the reasons the pollutant is expected to be discharged. <br />b) For every poflutam exported to be discharged in concentrations of 10 mg/P or greats the applicant must report quantitative data. For acrolein, <br />acryionitrilq 2,4 dinitrophenol, and 2-methyl-4,6 dinitrophenol, where any of these four pollutand are expected to be discharged im concentrations <br />of 100 Q}g/P or greats the applicant must report qualitative data. For every poflumnt expected to be discharged in concentrations less than ]0 <br />dyg/P, or N the case of acrolein, acrylonitrile, 2,4 dinitrophrnol, and 2-methyl-0,6 dinitrophrnol, in concentrations less than 100 mg/P, the <br />applicant must either submit quantitative dam or briefly describe the reasons the pollutant is expected to be discharged. <br />c) The applicant need not provide quantitative date if the pollutant is present in the discharge solely as the resuh of its presence in intake water. <br />I-Ioweva, the applicant must repot such polluant as present. <br />Item 31 - Please submit a copy of summaries of ell water quality dam performed in the previous five years under conditions of the permit from the Colorado <br />Division of Minerals end Geology. Such monitoring is often submitted a9 part of the annual report to CDMG and may consist of both oulfefl sampling <br />and instream sampling. If such monitoring is not required, Please so sdte. <br />Item 323 - Self Explanatory. <br />Item 34 - Describe those measures taken to control pollutand from entering wastewater streams.
