1 <br />' 1.6 Provisions of the Plan <br /> The following statements declare the intention of the facility management to comply with <br /> the applicable regulations as signified by management approval shown in Section 1.0 of <br /> this plan. <br />' The facility will, in accordance with 40 CFR 112.3, maintain a copy of this plan at the <br /> facility. The plan will be available for review during regular business hours upon request <br /> of the regional EPA administrator for Region VIII. The plan will be reviewed every 3 <br /> years as required by 40 CFR Part 112.5. The plan will be revised as necessary under 40 <br /> CFR Parts 112.4 and t 12.5. The plan will be re-certified by a Registered Professional <br />' Engineer after each revision. Discussion of revisions, amendments or updates aze <br /> attached as Appendix B. The plan has been certified by a Registered Professional <br /> Engineer as required by 40 CFR part l 12.3 and as shown by the signature and stamp in <br />' Section 1.0 of this plan. <br /> A designated person at the facility will be responsible for the plan as required by 40 CFR <br />' Part 112.3. <br /> As required by 40CFR Part 112.7, facility personnel are briefed on the plan at least <br />' annually. Records are maintained of plan briefings and are available for review at the <br /> facility. Records are retained for 3 years. During the annual plan review, personnel are <br /> instructed on spill response procedures and other safety related issues. <br /> <br /> Daily inspections are conducted for all of the areas described in this plan and the <br /> inspection records are kept on file in the facility records for three years. Appendix A <br />' contains blank copies of the inspection forms for each of the areas described in this plan. <br /> There are no internal heating coils employed at the facility, thus, monitoring and <br />' inspection controls are not necessary. All other sites can be inspected visually. <br /> The facility does not generally discharge effluent into navigable waters. The facility has <br />' applied for a Process Water and Stonnwater Discharge Associated with Sand and Gravel <br /> Operations permit. This discharge permit application includes a Stotmwater <br />' Management Plan (SWMP). <br /> The facility operates during daylight hours only. Yazd lights will be installed at the <br />' surface facility area to provide adequate lighting for nighttime security purposes. A fence <br />along the south property line separates the project area from the frontage road. The <br /> entrances to the gravel pits and the surface facilities are locked nightly. <br /> <br /> <br />' 6 <br />
