Exhibit C - Mininq Plan

5. In section al, include a proposed start date, (October 1 is the date you used in the notices).
6. In section bl, you must commit to seed the topsoil stockpile if the piles are not replaced or
naturally revegetated within one year.
7. In section e), if a crusher and/or wash plant are to be used, these facilities must be described.
Your plan for excavation must be described here. If your plan is to only excavate the existing
highwall so the slopes are laid back to 3H:1 V, and sell the resulting material, with no other
excavation planned, you must specify that plan here. If you will store hazardous materials
such as fuel, oil, antifreeze, etc. on site, you must commit to store them in an adequate spill
containment structure and describe that structure in this section. The structure must be
adequately sized to hold the contents of the largest tank to be stored within it.
8. In section f), you must include the maximum size of any disturbance areas other than the pit.
These areas include stockpile and facilities areas, areas to be stripped of topsoil, etc. Be sure
to describe any disturbance that existed prior to the gravel pit, and which is not related to the
mining operation.
9. In section g), please specify how long the road out of the pit is, and indicate if it will be
reclaimed or if it will remain as permanent access to the pond.
10.In section i), a well permit will most likely be required by the Colorado Division of Water
Resources if the operation exposes ground water. In this section, you must commit to not
exposing groundwater prior to obtaining a well permit for this operation.
11.In section 11, explain how runoff from the processing/stockpile area and roads will be handled
to minimize off-site impacts due to sedimentation or erosion.
Exhibit D -Reclamation Plan
1 2.In section a), please explain how excess overburden, that is not sold or used in the dam
construction, will be disposed in the permit area and reclaimed. Remember, no activities
related to the mining operation, including reclamation, can take place outside the permit
boundaries. It is suggested that the excess overburden be spread in a Payer inside the pit, and
then be compacted.
13.In section cl, the reclamation plan is unacceptable. please contact the Natural Resource
Conservation Service for recommendations that will result in establishment of a diverse,
beneficial, desirable vegetative stand that is capable of controlling erosion and is consistent
with the post-mining land use -dry rangeland.
14.In section dl, provide details and dimensions of how the dam will be constructed to ensure
long term stability. Include details for any inlet and outlet structures, and specify where the
water to fill the pond will come from lis it just acatch-basin type stock pond?1.
15.In section d), specify which roads will be reclaimed and how they will be reclaimed. It is
recommended that they be ripped prior to covering them with topsoil or overburden.
