E. REPORTING REQUIItEMENTS (cont.)
[' • 3) A summary of the annual inspection reports, including date, findings, and aaion taken, or the triennial
certification by a Professional Engineer (based on an inspection) that the facility is in compliance with the
permit.
4) Certification language and signature by the permittee. (See Part I.E.2 of the permit.)
The report will be due to the Division on or before February 15, 1999 and cover the period of time between the
coverage by this general permit and December 31, 1998. The Division reserves the right to require additional
information in the report, and more frequent reports, on a case-by-case basis, as needed.
Annual Report for Remediation Operations: The percnittee will be required [o submit an Annual Report,
covering January 1 through December 31 of each yeaz, on [he overall compliance with the SWMP. The Annual
Report shall contain, a[ a minimum:
I) Name of permittee, address, phone number, and permit certification number.
2) A report on the facility's overall compliance with the SWMP, including a progress report on all
remediation activities.
3) A summary of each comprehensive s[ormwazer facility inspection made, including date, ftndings, and
action taken.
4) Results and interpretation of any stormwater monitoring performed.
5) Certificazion language and signature by the permittee. (See Part I.E.2 of the permit.)
The report will be due [o the Division on or before February 15, of each yeaz. The exact due daze for the
pertnittee's first Annual Report will tie listed in thew permit certification. The Division reserves the right to
require additional information in the report, and more frequent reports, on a case-bycase basis, as needed.

a. All reports required for submittal shall be signed and certified for accuracy by the petmittee in accordance with
the following criteria:
1) In the case of corpora[ions, by a principal executive officer of at least the level of vice-president or his or
her duly authorized representative, if such representative is responsible for the overall operation of the
facility from which [he discharge described in the form originates;
2) In the case of a partnership, by a general partner;
3) In the case of a sole proprietorship, by the proprietor;
4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking
elected official, or other duly authorized employee.
b. Changes to authotvetion. ]f an authotvation under paragraph a. of this section is no longer accurate because
a different individual or position has responsibility for the overall operation of the facility, a new authorization
satisfying the requirements of paragraph a. of this section must be submitted to the Division, prior [o or
together with any reports, information, or applications to be signed by an authorized representative.
