PERMIT, PART I <br />Page lj of 13 <br />Permit No. CO.0034142 <br />C. STORMWATER REQUIREMENTS ' <br />i. An inventory and narrative description of significant materials that have bcen treated, stored or disposed of in a <br />mamer to allow exposure to stormwater from January 1, 1990 to the present; method of on-site storage or disposal; <br />and materials management practices employed to minimize contact of these materials with stormwater runoff between <br />January 1, 1990 and the present. <br />ii. A list of significant spills and significant leaks of toxic or hazardous pollutants that occurred at the facility from <br />January 1, 1990 to the present. <br />iii. For each mining site area that generates stormwater discharges with a reasonable potential for containing pollutants, <br />identification of the potential pollutants which ere 1'rkely to be present in the stormwater discharges. <br />iv. A summary of any existing discharge sampling data describing pollutants in the stormwater discharges. <br />e) Stormwater Ouality Controls. The SWMP shall include a description of stormwater quality controls to be implemented at <br />the Eagle Mine Complex. The control measures shall reflect identified potential source:; of pollutants at the site. The <br />description of stormwater quality controls shall address the following minimum components, including a schedule for <br />implementing such controls: <br />SWMP Administrator -The SWMP shall identify a specific individual or individuals within the mining organization <br />who is responsible for developing the SWMP and assisting the mine operator in its implementation, maintenance, and <br />revision. <br />ii. Risk Identification and Assessment -The SWMP shall assess ffie potential of pollutant sources at the mine to <br />contribute pollutants to stormwater discharges associated with mining activity. Each of the following shall be <br />evaluated for the reasonable potential of contributing pollutants to runoff: <br />- loading and unloading operations; <br />- outdoor storage of chemicals or equipment; <br />- outdoor milling or processing activities; <br />- historic mine drainage; <br />- tailings ponds; <br />- crushing facilities or significant dust and paRiculate generating activities; <br />- on-site waste disposal practices; and <br />- stockpiles of overburden, raw material, intermediate products, byproducts, finished products or waste products. <br />Factors to consider include: the tonicity of the material; quantity of chemicals used; amount of material processed; <br />[he likelihood of contact with stontrwater; and history of significant drainage, leaks or spills of toxic or hazardous <br />pollutants. In each case where stormwater pollution potential exists, appropriate preventive measures must be <br />identified and implemented. <br />iii. Preventive Maintenance - A preventive maintenance program shall be included. The program shall involve <br />maintenance of stormwater management devices (e.g., maintenance of dikes separating mine drainage from <br />stormwater, cleaning oil/water separators and catch basins, etc.), as well as maintenance of equipment and systems to <br />prevent conditions that could cause breakdowns or failures resulting in discharges of pollutants to surface waters. <br />iv. Good Housekeeping - Good housekeeping requires the maintenance of a clean, orderly facility. This paR of the <br />SWMP should address cleaning and maintenance schedules, trash disposal and collection practices, grounds <br />maintenance, etc. <br />
