The Colorado Registered Professional Engineer shall seal the.plans in accordance with <br />CRS I_'-'_S.I 17 in addition on the first page of the design plans that the followin note is required: <br />"This design is in full compliance With Section 4 Of the State Highway A <br />Code, ; CCR 601-1 except for the follrnving approved variances:' <br />Note: }t.LL plans must he submitted on 11" x 17" sheets at a minimum scald of <br />I" = 2©'. No other plan sheet sizes are authorized. The "As Buili' Plans must bf sealed in accordance with CRS <br />12-25-117. <br />25. It shall be the responsibility of the Permittee to verify the location of the existing utilities and notify all utility <br />owners or operators of any work that might involve utilities within the State Hig way right-of-way. Any work <br />necessary to protect existing permitted utilities, such as an encasement will be [h responsibility of the Permittee. <br />Any damage or disruption to any utilities during the construction shall be the Pe illee's responsibiliq• and shall <br />be repaired Ur replaced at no cost to the Department. <br />26. Any damagg to any presem highway facilities including traffic control devices s all be repaired immediately at no <br />cost to the f~epanment and prwr to continuing other work. Any mud or other m serial tracked or otherwise <br />deposited on the roadway shall he removed daily or as ordered by the Depar[me inspector. <br />27. Areas of roadway and/or right-of-way disturbed during this installation shall be r stored to their original <br />conditions, t,o insure proper strength, drainage and erosion control. <br />28. Any incomplete construction activity on the State Highway that must be left ove fight, shall be barricaded and <br />signed in accordance with the Manual on Uniform Traffic Con[rol Devices and o her applicable standards. <br />29. Open cuts, which are 6 inches in depth, within 30 feet of the edge of the Stale Hi hway traveled way, will not he <br />left open at Night, on weekends, or on holidays. <br />30. No more than 6 feet of trench arras shall be opened at any one time. Open trenc les and other excavations within <br />the State Hi4~hway right-of-way shall be backfilled and/or paved before 3:30 P.M of each working day or he <br />protected in'accordance with the M.U.T.C.D. <br />31. The area around the new work shall be well graded to drain, top soiled, fertilized, mulched and re-seeded in <br />accordance with the Department standard specifications. <br />32. When it is n@cessary to remove any highway right-of-way fence, the posts on eit r side of the access entrance <br />shall be securely braced with approved end posts and in conformance with the D partment's M-607-1 standard. <br />before the felnce is cut, to prevem slacking of the remaining fence. All posts and ire removed shall be returned to <br />the Department. <br />33. All excavations for utility lines, culverts, trenches or tunnels shall meet the requir menu of the Occupational, <br />Safety' and 1-(ealth Administration (OSHA), Colorado Industrial Commission, Col rado Division o(Mines or the <br />Colorado Department of Transportation, whichever applies. <br />34. All work that requires traffic control shall be supervised by a registered professio al traffic engineer or by a traffic <br />control supervisor certified by the American Traffic Safety Services Association ATSSA) or the Colorado <br />Contractors Association (CCA). When flagging personnel are required, the contr ctor in accordance with the <br />Department Standards shall certify them. <br />35. Consiructionl traffic control devices, when not in use, shall be removed or turned way from traffic. <br />36. All temporarti• pavement striping shall be done by the Permittee/contractor in con rmance with section 627 of The <br />Department'$ standard specifications for Road and Bridge Construction (latest ed lion). <br />L_J <br />J <br />• <br />l- <br />
