D) A letter from the OSE stating that the applicant has satisfied the OSE requirements for a well
permit and an augmentation plan or substitute supply plan for the proposed exposure of ground
water and evaporative loss.
~~ representative cross sectional design of the liner construction, design st ards, ~a/~~ i,
alive of cons cti n methods to be se to construct a liner.." ~ ~~ /
EXHIBIT E - CLAMATION PLAN ,ff ~ 7" S G/~C~,
J 9. The applicant states on page 17 that topsoil will be replaced on all areas above th8'anticipate~ti ~~ QO
high water line at a minimum thickness of 12 inches. Accordingly, the Division requests [hat t e „_/ ?
applicant define the volume of topsoil that will be maintained on site for reclamation and the ~ ~~
acreage of topsoiled areas that will be revegetated with a grass seed mix. ~ _ ,
~10. As required by Rule 6.4.5(2)(f)(i), please specify the desired slope gradient (if any) to be
achieved for the wetland benches.
~ 7 x.~ao x ~f~ ~~.r~2
11. There are no provisions for revegetation of the 7 acres of wetlands benches. Although a U.S.
Army Corps of Engineers 404 Dredge & Fill Permit may not be needed for created wetlands, the
applicant will still need to provide the Division with a revegetation plan for the created wetlands
and itemized costs for backfilling & grading as well as revegetation of such azeas. (If the
applicant is planning on 100% volunteer growth of wetland vegetation, the Division will still
require a contingency plan and cost for seeding).
I N'1i~
12. As required by Rule 6.4.5(2)(f)(iv), please specify the approximate diameter and number(s) is ~ ~:~~
of trees and/or shrubs to be planted during final reclamation. '~~,D tXrO ~~~°°
/ ~~ v
V 13. Because of the location of the proposed slurry wall, there may be a slight alteration of the
existing surface and groundwater drainage pattern (recharge/dischazge) in the cottonwood grove
between the South Platte River and Ponds A & B. Therefore, the potential exists for adverse
impacts to the mature cottonwoods at several or more times throughout the year. Accordingly,
the Division requests that the applicant provide additional details of the monitoring and
mitigation to be done to minimize possible impacts to the cottonwoods throughout mining and
reclamation. Specifically, what parameters will be monitored, what is the frequency of
monitoring, will results be reported to the Division and at what frequency, what are the specific
"triggers" that will define when the trees are being stressed, and how long will supplemental
irrigation water or groundwater from the pits be applied in the event of a possible impact.
EXHIBIT G -WATER INFORMATION
~1 14. Both the Existing Conditions Map and Mining Plan Map show the locations of 10 existing
~ monitoring wells. Please explain the type & configuration of the wells and nature of any on-
going or proposed monitoring program. `~ ~ /, J,`~^ ~r 1 7
~>
