Based on the Division's pre-operation site inspection on July 26, 2000 it is <br />appropriate and necessary for the applicant to include in the Milliken Site application a weed <br />control plan with specific provisions for the various thistle species present. The reclamation cost will need to be revised to include itemized costs for the following: <br />slurry wall installation (10,388 lineaz feet) /0~31~D <br />pit dewatering costs (assume active mining area during maximum disturbance is full of water) <br />contingency costs for liner repair (assume 20% of entire reservoir perimeter will need to be <br />ripped, relined with replacement clay/shale, and recompacted to O5E design specifications) <br />equipment mobilization demobilization (specify # pieces of equipment) <br />EXHH3IT M -OTHER PERMITS & LICENSES <br />V 18. In the event that a 404 Permit is required from the U.S. Army Corps of Engineers for any <br />existing or constructed wetlands, please provide a copy of the approved application as it becomes <br />available. <br />EXHIBIT S -PERMANENT MAN-MADE STRUCTURES <br />19. Christina Kamnikaz of the Division is reviewing the stability analysis provided for the <br />various structures located inside or within 200 feet of the affected land and her comments will be <br />forwarded to the applicant on or before August 25, 2000. Although not specifically required, the <br />Division suggests that the applicant provide a list of the various structures inside or within 200 <br />feet of the affected ]and. The following information would be helpful: name or type of structure; <br />name, address, & phone number of owner of structure; recommended setback distance from <br />structure; whether structure will remain or be removed during mining. <br />
