Poly-Flex continued deployment of 80-mil LLDPE geomembrane in the Phase IVa azea. Poly-Flex
deployed 80-mil single-sided textured LLDPE geomembrane over an azea of 298,459 square feet in
the Phase IVa azea during this reporting period. Geomembrane panels P370 through P470 were
deployed, seamed, and tested by Poly-Flex. Destructive fusion seam samples DF-133 through DF-
164, and extrusion seam samples DX-8 and DX-9 were marked, removed, and tested.
Ames continued placement of Drain Cover Fill (DCF) and haul road construction in the Phase IVa
area. The DCF was placed using CAT D6, D8, and D9 dozers. The DCF was placed on the floor and
slopes, advancing from north to south.
Ames began final grading of the Phase IVa diversion channel by grading subgrade according to
design drawings and placing 6-inches of low permeability channel lining fill.
Ames continued construction of the three (3) 42" CSP's for the VLF access road by placing the CSP's
and backfilling azound them.
Phase Nb
Ames continued placement of Structural Fill (SF) in the Phase IVb azea. Ames placed SF above the
Phase III diversion channel, at the old haul road below the substation, and at the western end of the
PSSA floor. Ames also continued slope recontouring in the Phase IVb azea on the northern PSSA
slope and on Chicken Hawk Hill.
Ames hauled oversize material from the Phase Nb azea to an uncontrolled fill site.
Ames performed confirmatory drilling activities associated with the Phase Nb construction area on
underground workings UG-493, the Ophir tunnel, UNK-I1, and UNK-16.
Ames performed excavation remediation activities for underground workings UG-215, UG-263, UG-
264a, UG-264b, UG-546, UNK-11, UNK-13, UNK-14, and UNK-16. These workings were observed
and verified to be "Shallow or Deep", as defined in the drawings, and will be remediated accordingly.
Ames conducted blast drilling of underground workings within Phase Nb at UG-215, UNK-15, and the
Ophir tunnel. Ames blasted underground workings delineated by confirmation drilling at UNK-15 on
July 29a' at 17:00, at UG-215 on July 3151 at 17:00, and at the Ophir tunnel on August 8a' at 17:00.
Ames resumed placement of Concrete Plug (CP) concrete into "Deep Shafts" UG-214, UG-215, and
UNK-I5. Ames placed three feet of concrete in accordance with IFC Drawing 12, Detail 1.
Ames resumed placement of Select Structural Fill (SSF) into "Shallow" workings at UNK-15.
Ames used Structural Fill to backfill "Shallow" working UNK-14.
Phe,e1Ve 59lu1y 25-Augus~l0.doc Golder Associates lnc.
