Ames constructed a geogrid cap over UG-546. Ames placed two layers of geogrid in accordance with <br />the drawings and specifications. <br />I ~ Ames did not perform confirmation drilling in the Phase Nb area during this reporting period. <br />Phase Nc <br />Ames continued placement of Coarse Shaft Backfill (CSB) into "Deep" underground working UG-504. <br />(See Attachment A for approximate construction locations for all Phase IV areas.) <br />Field Monitoring and Testing Activities <br />Golder observed construction activities associated with Phase IV earthworks. All construction <br />materials and soil materials placed were verified as suitable, and construction methods were in <br />accordance with project specifications. Removal of oversized and deleterious material from all fill <br />areas was observed. <br />Golder observed recontouring of the Phase III ore slope. Ore was regraded to approximately 4H:1 V <br />and at no time was allowed off of the existing liner system. <br />Golder observed construction of the Primary Underdrain Sump in Phase Na complying with Details <br />5/13 and 6/13 of the IFC Drawings. The portions of the anderdrain sump inlet and sump that were <br />completed were constructed in accordance with the drawings and specifications. <br />Golder observed construction of the Phase Na Leak Detection trench for trench dimensions, <br />geomembrane welds, geotextile overlap, and LDF. The Leak Detection Trench was constructed in <br />accordance with the drawings and specifications. <br />Golder observed and tested placement of SLF in Phase Na, completing twenty-three in-situ density <br />tests, six depth verification tests, and verified surface preparation. Prior to geomembrane <br />deployment, the SLF met the requirements of the specification. <br />Golder observed construction of the Temporary and Permanent Diversion channel for proper <br />materials, thickness, grades, and compaction. The Temporary and Permanent Diversion channel <br />activities were performed in accordance with the drawings and specifications. <br />Golder observed geomembrane deployment, documenting ambient air temperature, panel <br />dimensions, seaming, tie-in overlap, and seam testing (trial seams, pressure, vacuum, and <br />destructive). Geomembrane placed in Phase IVa met specifications. <br />Golder observed and documented excavation and remediation activities associated with the Phase IV <br />underground working construction. Full time monitoring was given to excavation of underground <br />workings. <br />Golder observed placement of CSB. Coazse Shaft Backfill was placed in accordance with the <br />rn.,~ rva ss rte:-~s.ao~ Golder Associates Inc. <br />
