L <br />' Scow of Woh <br /> <br /> <br /> <br />1 <br /> <br /> <br /> <br /> <br /> <br /> <br />1 <br /> <br /> <br /> <br /> <br />1 <br /> <br />All four monitor wells will be installed with <br />20 feet of two-inch diameter, flush joint, <br />0.020-inch machine slotted PVC well screen. <br />Each boring will be completed with 10 feet <br />of the screen in the saturated zone. This <br />depth will ensure a sufficient water column <br />to accommodate groundwater potentiometric <br />surface fluctuations. Blank casing of similar <br />construction will be added to reach <br />approximately three feet above surface grade. <br />The bottom of each well will be fitted with <br />a two-inch diameter PVC bottom cap, and <br />the top of each casing will be fitted with a <br />locking gripper cap. The annulaz space of <br />each well was filled with silica sand to a <br />level of two feet above the screened interval. <br />A two-foot layer of bentonite will be <br />emplaced above the sand, preventing surface <br />infiltration. Concrete grout will be added to <br />reach neaz surface grade where an above- <br />ground metal protective wellhead cover will <br />be emplaced to complete each well. Well <br />completion logs will be submitted to the <br />Division by February 16, 1996. <br />Condition 6: The proposed four monitoring <br />wells will be drilled, completed, logged and <br />tested within forty-five (45) days of the <br />beginning of the drilling program, which will <br />commence no later than March 1, 1996. <br />Work Plan: Greystone will be able to log <br />and test the wells within forty-five (45) days <br />after the beginning of the drilling program. <br />Greystone has identified and received a <br />commitment from a driller meeting the <br />required specifications that drilling for this <br />project will commence no later than Mazch <br />1, 1996. All four monitoring wells will be <br />completed according to specifications <br />outlined as well as the required logging and <br />testing within 45 days of the Mazch 1, 1996 <br />deadline. <br />Condition 7: Drillers, lithologic and <br />geophysical logs will be provided for the <br />four monitoring wells within 15 days after <br />the logging of the last of the monitoring <br />wells has been completed and the depth of <br />the saturate groundwater zone will be <br />indicated on these logs. <br />Work Plan: Lithologic and geophysical <br />logs will be provided for the four monitoring <br />wells within 15 days after the logging of the <br />last well. <br />Condition 8: When the monitoring wells <br />aze accepted as ready for sampling, <br />groundwater samples will be obtained from <br />them at least one time each calendaz quarter. <br />Work Plan: Following Division approval of <br />the well installation, Greystone will collect <br />groundwater samples following the standard <br />operating procedures (Appendix I). <br />Condition 9: After five (5) calendaz <br />quarters of sampling, the County will submit, <br />for Division approval as a proposed <br />Technical Revision, permit conditions for <br />groundwater at the site and Point(s) of <br />Compliance satisfying the requirements of <br />Rules 3.1.7(2) and 3.1.7(6) of the <br />Construction Materials Rules and <br />Regulations. The proposed permit conditions <br />and Point(s) of Compliance will be <br />submitted to the Division within fifteen days <br />following receipt of the analyses of the <br />samples taken during the fifth calendar <br />quarter of the sampling program. <br />Work Plan: Following receipt of the fifth <br />quarter analyses, Greystone will prepaze <br />Technical Revisions, Permit Conditions for <br />groundwater at the Site and Point(s) of <br />Compliance which will continue to satisfy <br />Costillal I-96 <br />2-3 <br />
