PART I <br />Page zii <br />Permit No. CO-0043648 <br />If the permi[tee, using the approved analytical methods, monitors any parameter more frequently than required by Ihis <br />permit, then the results of such monitoring shall be included in the calculation and reporting of the values required in <br />the Discharge Monitoring Report Form or other forms as required by the Division. Such increased frequency shall <br />also be indicated. <br />See Part I.C. for Definitions. <br />2. Rye ortinc of Data <br />Reporting of the data gathered in compliance with Part I.B.1 shall be on a monthly basis. Monitoring results shall be <br />summarized for each month and reported on Division approved discharge monitoring report (DMR) forms (EPA form <br />3320-1). The forms shall be mailed to the agencies listed below so they an; received no later than the 28th day of the <br />following month. if no discharge occurs during the reporting period, 'No Discharge" shall be reported. <br />The DMR forms consist of four pages -the top 'original' copy, and three attached nocarbon-required copies. After <br />the DMR form has been filled out and signed, [he four copies must be separated and distributed as follows: <br />The first on final signed copy of each discharge monitoring report (DMR) shall be submitted to the Division et [he <br />following address: <br />Colorado Department of Public Health and the Environment <br />wQCD-PE-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80222-1530 <br />The first duolicate signed copy of each discharge monitoring report (DMR) shell be submitted to the following <br />agency: <br />U.S. Environmental Protection Agency <br />Water Management Division <br />NPDES Branch SWM-C <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />The third and fourth copies are for the petmittee records. The Discharge Monitoring Repoli forms shall be filled out <br />accurately and completely in accordance with requirements of this permit and the instructions on the forms. They <br />shall be signed by an authorized person as identified in Part I.D. <br />3. Special 111onitorin~ <br />a. Discharges From Sources Not Specifically Identified to The Permit <br />For any point source discharge of process water or combined process water/s[otrowater that has or may reach <br />surface waters of the State, that is not specifically identified in the tables of effluent limitations or monitoring <br />conditions listed in Part I.A.1. or I.B.I. of this permit, the permittee shall initiate sampling and analysis of that <br />discharge as soon as it is discovered. <br />The parameters to be analyzed for are those listed in Table I.B.-l. Monitoring of such discharges shall continue <br />at the frequency shown in Table I.B.-1 until either [he discharge is eliminated or the Water Quality Control <br />Division notifies the petmittee in writing that the monitoring frequency may be reduced, or must be increased. <br />INDBP aN9l <br />
