B.-2, and <br />any observable damage to the liner must be repaired. <br />3 Water management must be improved in order to reduce the leakage rate. Special notification must be provided <br />to the Water Quality Control Division and [he Division of Minerals and Geology indicating what the leakage <br />rate Is and what measures have been taken to reduce it. The permittee shall provide verbal notification the <br />WQCD .vithin 24 hrs, and written notification within 5 days. <br />4 Additional measures must be implemented in order to reduce leakage, which may include the inspection and <br />repair of [he liner or the retluclion of head over [he liner. Special notification must be provided to the Water <br />Quality Control Division and the Division of Minerals and Geology indicating what the leakage rate is and what <br />measures have been taken to reduce it. The permittee shall provide verbal notification to the WQCD within 24 <br />hrs, and written notification within 5 days. <br />In addition to the notification provisions listed above, when cyanide is detected for points 002A or 003A, the <br />permittee shall provide verbal notification to the WQCD within 24 hrs, and written notification within 5 days. <br />Self-monitoring sampling by the permittee for compliance with the monitoring requirements specified above shall be <br />performed at the locations listed in the tables. <br />INUHY arVN] <br />
