All of these elements <br />would be incorporated into each exhibit section. The Division would carefully <br />review the old permits to ensure that previous requirements were no"t <br />overlooked. <br />5. The amendment would include some of the following areas. This "fist is not <br />all inclusive. <br />Exhibit A - <br />A revised legal description for the entire permit area. Acreage pr~aviously <br />included would be included here. Deletion of acreage could only occur if the <br />land was never disturbed. <br />Exhibit B - <br />Submit a revised index map. <br />Exhibit C - <br />This exhibit is very important. Several maps would be submitted. One map <br />would clearly show the new permit area in relation to the old permit areas. <br />Additionally, your maps will show existing and projected pit areas, roads, <br />storage and office areas, topsoil locations, and all other features as <br />required under Rule 2.12(3). Scales would best be in the 1" = 200' or 1" _ <br />300' range. The maps should be well designed to clearly portray your plans <br />and conditions and to allow easy field identification of features. Show the <br />actual permit boundary clearly. <br />Exhibit D - <br />Provide details as per Rule 2.12(4) incorporating past, present and future <br />information as previously discussed. We will require some form of <br />documentation that agreements exist between the operator and owners of <br />structures within 200 feet of any affected land (such as irrigation ditches, <br />roads, power lines, gas lines etc.) which clearly accounts for any damage <br />incurred to the structure as a result of mining. (See section 34-32-115(4)(d) <br />of the "Act") <br />Exhibit E - <br />Revise the reclamation plan as with the other exhibits.
