6. Please submit a certificate showing personal injury and property damage insurance is in <br />force as required by Rule 2.03.9. <br />Northfield Response: Insurance certificate submitted May 10, 2007 . <br />DRMS response: The insurance certificate is not acceptable in its present form. Text on <br />page 2.03-13 refers to a rider ensuring notification to the Division of termination or changes <br />in the policy. No such rider was found in the materials submitted. Furthermore, the <br />cancellation clause on the certificate mentions 0 days notice will be mailed to the Division. . <br />As a matter of policy and practice, 30 days notice is the common period for sending written <br />notice or changes or policy termination. Please refer to Rule 2.03.9. Please submit a valid <br />insurance certificate striking the words "endeavor to" and all words including and <br />after "but failure to do so...". <br />7. Please update the information submitted under Rule 2.03.10. <br />Northfield Response: Air permit for construction and CUP submitted. <br />DRMS response: Response accepted following the May 30, 2007 submittal. <br />Rule 2.04 Environmental Resources <br />8. The Horseshoe Mine is shown on Map 4 as historic mining within the permit area but is not <br />ident~ed (other than in the table) in permit text. This historic mining is very close to <br />privately owned surface lands. Please ensure that information on all historic mining <br />activity within the proposed permit area is included as required by Rule 2.04.3(3). <br />Northfield Response: Horseshoe mine is now outside permit area. <br />DRMS response: Please "label" the permit boundary on Map 4. <br />9. The Division is currently asking for additional clamcation from the State Historic <br />Preservation Office on the potential need for surveying the proposed disturbed area for <br />cultural resources. If any additional information is required regarding cultural resources, <br />we will contact you. (Rule 2.04.4) <br />Northfield Response: Cultural surveys completed and submitted. <br />DRMS response: It is not cleaz from the text and map in the Metcalf survey that the <br />ventilation shaft area was included in the survey. Please submit documentation that the <br />vent shaft area was included in the survey or, if not included, submit survey <br />information for this area and the access road leading to the proposed shaft site. <br />10. Please submit the chemical analysis of the roof and ,floor in accordance with Rule <br />2.04.6(3)(a)(iii)(D) for the exploration hales drilled in 2005. <br />
