For information on land use, soils, geology, vegetation, fish and <br />wildlife, water quality and quantity, archeology, and air quality, <br />the applicant should provide a list of the name, address and posi- <br />tion of persons of each private or academic research organization <br />or governmental agency consulted in obtaining that information, <br />2.03.4 Identification of Interests <br />1. Are the listed owners of record surface owners, subsurface owners, <br />or both? <br />2. The applicant needs to identify what type of business entity the <br />applicant can be identified as. <br />3. Is the B.L.M. (i.e., The United States of America) the only owner <br />of record of all surface and subsurface areas contiguous to the <br />permit area? If not, the applicant needs to provide the names, <br />and addresses of such owners. <br />2.03.5 Compliance Information <br />1. The applicant must make a statement addressing revegetation or <br />suspension of a mining permit, or forfeiture of a mining bond for <br />coal mining as outlined in 2.03.5(1). <br />2.03.6 Right of Entry and Operation Information <br />1. In the right of entry information, H.E. Lowdermilk Co. and Flatiron <br />Paving Co. have been given entry and operation permission but Walden <br />Coal Co. is not mentioned. The applicant needs to clarify this. <br />2. Has the private mineral estate been severed from the private sur- <br />face estate? If so, the applicant must address the requirements <br />in 2.03.6(2). <br />2.03.9 Personal Injury and Property Damage Insurance Information <br />1. The applicant must provide minimum insurance coverage for property <br />damage of $300,000 for each occurrence and $500,000 aggregate. Cur- <br />rent insurance information indicates property damaye coverage of <br />$250,000. <br />2.04.3 Site Description and Land Use Information <br />1. The applicant should provide a discussion of the condition, capa- <br />bility and productivity of the land within the permit area. <br />
