Stover & Associates -3- May 31, 1996 <br />Bowie No. 2 Mine <br />2.03.9 Personal Iniury and Property Damage Insurance Information <br />Prior to receiving approval to conduct coal mining operations, a <br />copy of the personal injury and property damage insurance must be <br />filed with the Division. <br />Please submit a copy of the personal injury and property <br />insurance which contains minimum coverage of $300,000 for each <br />occurrence and $500,000 aggregate for both property and <br />personal injury. <br />2.03.10 identification of Other Licenses and Permits <br />it is understood that applications are pending for both air <br />emission and wastewater discharge permits, however, this item is <br />still going to be added as an adequacy question to ensure that this <br />process is properly tracked and that the permits are submitted as <br />required. <br />Please provide copies of the Air Emission Permit and the <br />Colorado Wastewater Discharge Permit upon issuance, along with <br />any other required permits such as well permits or local land- <br />use permits, in a format which can be easily inserted into the <br />permit application document. <br />If any wetlands are to be disturbed, it is the Division's <br />understanding that a Department of the Army 404 dredge and fill <br />permit will be required. <br />2.03.10 Location of Public Office for filing of Application <br />No response required. <br />2.03.12 Newspaper Advertisement and Proof of Publication <br />The proposed newspaper advertisement is adequate. The Division <br />received notice of proof of publication for this application on <br />April 11, 1996. <br />22.04.3 General Requirements: Bite Description anfl land Use <br />Information <br />It will become very important to both the operator and the Division <br />as to extent and location of the acreage disturbed by previous <br />mining operations when reclamation operations begin in the future <br />after mining under this permit has ceased.
