a~ ,i <br />Chimney Rock Coal <br />Martinez Strip Preliminary Adequacy Review <br />RULE 2.03 APPLICATION FOR PERMIT FOR SURFACE COAL MINING AND RECLAMATION <br />OPERATION: MINIMUM REQUIREMENTS FOR LEGAL, FINANCIAL, COMPLIANCE' <br />AND RELATED INFORMATION <br />Section 2.03.8 Permit Term Information <br />1. The applicant needs to state the anticipated starting and termi- <br />nation date of each phase of the operation. Included in this <br />should be the number of acres affected for each phase of the <br />operation and total acreage affected over the life of the opera- <br />tion. <br />I.E. phase 1: starting date 8/81, termination date 8/82; acres <br />affected 8. (2.03.8(1)) <br />Section 2.03.9 Personal Injury and Property Damage Insurance Information <br />1. The applicant's insurance policy expires 9/30/81. The applicant <br />must maintain a policy in full force over the life of the opera- <br />tion including completion of all reclamation activities. Also, <br />the policy must specifically cover damage incurred due to the <br />use of explosives. Since the policy will expire prior to the <br />Division's decision on this application, the applicant should for- <br />ward copies of the renewed policy or a new policy to the Division <br />for insertion into the application to meet the requirements of <br />2.03.9(1). <br />Section 2.03.12 Newspaper Advertisement and Proof of Publication <br />1. The applicant needs to supply proof of publication to the Division <br />no later than 4 weeks after the last date of the publication re- <br />quired under 2.07.3(2) <br />RULE 2.09 APPLICATION FOR PERMIT FOR SURFACE OR UNDERGROUND MINING <br />ACTIVITIES-MINIMUM REQUIREMENTS FOR INFORMATION ON EN- <br />VIRONMENTAL RESOURCES <br />Section 2.09.4 Cultural and Historic Resource Information <br />1. Information on Cultural and Historic Resources in the area was for- <br />warded to the appropriate state agency for review. Please find.a <br />copy of their findings enclosed. 2.04.4 , <br />Section 2.04.5 General Description of Hydrology and Geology <br />1. The applicant needs to provide a Regional Geology Map showing the <br />location of the permit area, and depicting any significant geo- <br />logic, geomorphic and structural Features of the region including <br />regional aquifers. This information is necessary to facilitate <br />an understanding of hydrologic systems within the general area of <br />the mining operation while showing the relationship between the <br />regional and site specific geology. A U.S.G.S. quadrangle map <br />with the appropriate information would be adequate. 2.04.5(1) <br />
