<br />(c) Provide a list of each violation notice, if any, re- <br />ceived by the applicant in connection with any surface <br />coal mining operation during the 3-year period before <br />the application date for any applicable Federal or <br />State laws as described under this paragraph. A state- <br />ment concerning each violation should include the in- <br />formation required in 782.14 (c)(1-5). <br />782.15 <br />(a) In addition to, and as a description of the documents <br />contained in Exhibit 0 of the permit application, pro- <br />vide the information, where appropriate, required by <br />782.15 (a), (b)(1-3). <br />752.16 <br />(a-c) The requirements specified by this section need to be <br />addressed. Criteria for limiting or prohibiting mining <br />are in Sections 701.11 and 762.11. Provide maps showing <br />available information pertinent to this section. (Can <br />include a checklist showing that an inventory of the <br />criteria has been made or, it appropriate, an explicit <br />statement saying "no such resources.") <br />782.17 <br />(a) State the anticipated starting and termination date of <br />each phase of mining. Describe the mine plan in terms <br />of portions to be mined at intervals of time over the <br />life of the permit (i.e., one-year phases.) <br />(b) Tne potential exists for mining beyond the normal 5-year <br />permit limit. If a longer permit term is desired the ap- <br />plicant must justify it under the provisions stated in <br />Section 786.L5 (a). <br />782.18 <br />Provide a certificate of liability insurance pursuant <br />to SUti.14 (a-c) or evidence of self-insurance as des- <br />cribed in 80ti.14 (d). <br />782.19 <br />Provide a list of all other licenses and permits needed <br />by the applicant in accordance with (a)-(d) of this sec- <br />tion. <br />'L <br />
