<br />771.27 <br />Include a statement, verified under oath by a responsible <br />official of the applicant, tttat the information contained <br />in the application is true and correct. <br />782.13 <br />(b) Provide a statement describing the status of Sunflower <br />Energy Corporation. <br />(1) Provide names and addresses of every officer, partner, <br />director, or other person performing a function similar <br />to a director of Sunflower Energy Corporation. <br />(2) Provide names and addresses of principal shareholders of <br />Sunflower Energy Corporation. <br />(3) Provide names under which the applicant, partner, or prin- <br />cipal shareholder operated a surface coal mine in the U.S. <br />within 5 years preceeding the application date. If no such <br />mines exist, provide a negative statement. <br />(c) Provide names and addresses of respective principals, of- <br />ficers, and resident agents of U.S. Steel Corporation. <br />(d) Provide a list of any current, previous, or pending coal <br />mining permits in the U.S. held by the applicant or those <br />identified in Paragraph 778.13 (b)(3) above subsequent to <br />1970. List by permit or permit application number and <br />identify regulatory authorities. <br />(f) Provide 1•ISHA number, if any. <br />(g) Provide a list, if any, of contiguous lands, interests <br />in lands, options, or pending bids on interests held or <br />made by the applicant for lands which are contiguous to <br />the area to be covered by the permit. <br />782.14 <br />(a) Provide a positive or negative statement as to whetner <br />the applicant „ any subsidary, affiliate, or persons con- <br />trolled by or under cormnon control with the applicant has: <br />(1) had a Federal or State mining permit suspended or revoked <br />in the last 5 years; or, <br />(2) forfeited a mining bond or similar security deposit. <br />(b) If the statement pursuant to 778.14 (a)(1,2) is positive, <br />provide information fulfilling the requirements of 782.14 <br />(b)(1-5).
