11) Annual Reclamation Reports are due by March 28 following (Permit, page 4.05-11). The most
recent Annual Reclamation Report (separate binder) on file (on a bookshelf) is 1 Y - ~

13) DMG/OSM inspection reports on file in cabinet 3 drawer 1. The most current inspection report

on file. A minimum of three years of consecutive reports previous to the most current (is) (ls not) on
file. Three years of DMG/OSM enforcement actions are required to be on file. The Iasi DMG/OSM
enforcement action was Issued on May 7, 1996. Thus no enforcement action needs to be on file.

15) Pond cerlfications are on file in the inspector binder. Ponds 001,002, 003, 005 are statements
dated b-264. Gulch A pond is a statement dated 11-1 ~5. Gossard Loop Pond 0061s a
statement dated 3388. Prospect Pond 007 is dated 12-9-92 The percolation and aeration ponds
are dated 6393. The washbay and sediment sump ponds are date April 29, 1994 (constn~ction
certfication 4/5/94). East Taylor pond is dated Oct 23, 1995. The West Pit pond is dated October
17, 1995. The Section 16 pond is dated December 1, 1994. Re~ertfication of the sedimentation
ponds was accomplished as a part of Technical Revision Number 41.

construction certifications are found in Etfiibit 19. The Streeter Ell construction certification is on
file as Map 30.
1 ba) Quarterly piemmeter readings and spring and seep surveys are required of the Sfreeter Fill
(Permit, page 4.09-1 ). Reports of these readings and surveys are filed in the inspector binder until
incorporated into an Annual Reclamation Report. The date of the last Sfreeter EII piezometer
