fill around the stacking tubes; 6/29/93 Annual certification of ponds MB-1, MB- <br />2, and FW-1 (These ponds meet the State Engineers sizing criteria requiring <br />annual certification. -; quarterly sed. pond inspections current thru 7/30/93 ( a <br />potential seep at the toe of MB-1 was noted in the pond inspection. A seepage <br />control plan is currently being developed by Envirocon for minewide application. <br />The plan will also address slides and slide mitigation. ); refuse pile inspection <br />reports were current through 7/30/93 (a small slide was noted during the third <br />quarter reporting period which originated on the unaffected northeast facing <br />slope adjacent to the 100yr./24hr. surface runoff ditch. The ditch has been <br />cleared and the permittee is awaiting the slide mitigation plan referred to above <br />before completing the approved terrace construction on the west facing slope <br />of the refuse pile. The mitigation plan should address how the terrace ties into <br />this potential slide zone. ); the general minesite inspection and topsoil/subsoil <br />stockpile inspection was conducted by the permittee 7/30/93.; original pond <br />certifications dated 2/3/81, 1/11/82, 8/9/84 and 8/19/86 were reviewed <br />Stipulation No. 45 in the 12/16/92 Permit Renewal document requires <br />resolution of all outstanding issues from the 1991 Permit Renewal. TR-64 <br />apparently addresses most of the issues and all of the hydrological control <br />systems deficiencies. Sediment pond deficiencies include :establish 1 ft. of <br />freeboard at pond MB-4; modify combined spillway at pond MB-6; and establish <br />dewatering device at pond MB-3. TR-64 is scheduled for approval on 8/20/93 <br />according to DMG. The permittee indicated they intend to submit a major <br />6 <br />
