01-19-83 <br />RP-4 certified on 11-26-90 <br />RR-1 and RR-2 certified on 11-29-84 <br />SS-1 and SS-2 certified on OS-22-90 <br />During the inspection several impounding structures <br />were noted that had not been identified during the <br />records review. This included an impoundment at the <br />railroad loadout, two impoundments at the conveyor <br />transfer station #1 and the impoundment (lagoon) used <br />to hold fresh water by the White River. These <br />impoundments have not been designed and/or certified <br />and were not being inspected on a quarterly basis. <br />DMG issued NOV #C-93-157. <br />Three impoundments at the East Portal sumps also fall <br />into this same category, however the company was <br />previously notified of the deficiency with these <br />structures and is presently working to bring these <br />into compliance. <br />Coal Mine waste: Refuse piles 2-3 and 4 were last <br />certified on 11-29-93. <br />Compaction test of the refuse piles was done only once <br />during the past year, July 1993. The last test before <br />that was in August 1992. There appears to have been <br />several lifts completed during the construction of the <br />refuse piles between testa Of the 8 samples taken <br />during the July test period, 6 did not meet the 90~ <br />compaction to dry density required by the regulations. <br />There is no record to indicate that the required <br />compaction was obtained prior to continuing <br />construction of the piles. This lack of information <br />supports the assumption that the stability of the <br />piles is questionable. <br />DMG issued NOV #C-93-156 for failure to compact coal <br />refuse to 90% of maximum dry density and to determine <br />if lifts of refuse had been compacted to 90~ of <br />maximum dry density. The abatement steps require the <br />company to submit a plan with a schedule for testing <br />compaction frequently enough to verify compaction. I <br />would suggest that in addition to the plan the company <br />verify that lifts previously completed meet the <br />compaction requirements. <br />6 <br />
