(Page 3)

MINE ID # OR PROSPECTING ID # M-94-027
INSPECTION DATE 9/26/2001

INSPECTOR'S INITIALS TAS
the original permit application). It appears that the original appliant may have intended
for the plant site area to be reclaimed to agricultural land. The Division considers this
matter to be a problem and requests that the operator clarify the plans for final
reclamation of the plant site via submittal of a permit modification. An application for
a technic a7. revision will be sufficient to resolve this problem if the operator will
reclaim the plant site area in a manner that is consistent with the designated post-mining
land uses and the reclamation plans for the surrounding areas. At a minimum, the
technical revision application should include plans and itemized costs for demolition of
foundations and permanent structures, ripping of compacted areas, and stabilization of the
affected land through measures such as topsoiling and revegetation. As an alternative,
the operator may wish to leave the plant site area intact to serve as an
industrial/commercial area. Under that scenario, the operator will need to submit an
application for a permit amendment that will include the following components: 1) written
verification from the landowner that they desire the plant site to be left as is for
industrial/~=ommercial purposes, 2) adequate demonstration that there will be at least one
or more outside sources of pit run aggregate to come into the plant site at the time of
final reclamation, and 3) written verification from the Weld County Planning & Zoning
Department that the plant site area in question is or will be zoned for
industrial/commercial use. See Rule 3.1.11 for further information. Please see page 4
for Corrective Actions.
10. Based on the observations noted during the inspection, it appears that the operator is
in compliance with the mine and reclamation plan and that the Distel Farm Resources operation
is adequately :oonded.
I & E Contact ilddress
^ CE
NAME Connie Nickle Davis ^ BL
OPERATOR Aggregate Industries ^ FS
STREET 3605 South Teller Street ^ HW
CITY/STATE/ZIP_ Lakewood.
