February 4, 2002 Letter from Division: <br />Permit Identification Sign: <br />The Division acknowledges the installation of a Permit identification sign at the Topaz <br />Mine. The permitted area boundaries have not been marked, so the Corrective Action item is <br />still open. <br />Response: <br />As noted in the preceding comments, the Operator is ready to mark Affected Area <br />boundaries upon reaching concurrence with the Division on the Proposed Affected Area <br />boundary as shown on the 1" = 100' map submitted with the Annual Report. <br />Removal of Oil Barrels: <br />The Division acknowledges the removal of the waste oil barrels and the Corrective <br />Action is satisfied. <br />Response. <br />No response. <br />Revised Maps: <br />The outstanding Corrective Action requiring the submittal of revised maps has not yet <br />been addressed. <br />Response: <br />A revised map was submitted with the Annual Report on January 28, 2003. <br />October 7. 2002 Letter from Division: <br />Description of Site Features: <br />A complete description of all site features is required so that determinations can be <br />made about which historical features can be eliminated from the list of reclaimable <br />disturbances. Descriptions of pre-Law features must be adequate to determine if hazards <br />exist. <br />Response: <br />The Annual Report submitted on January 28, 2003 describes the site features. To the <br />best of this Operator's knowledge, there are no historical or pre-Law features at this site or <br />within the Permit area (there are old mine sites within the region, but these are not on lands <br />controlled by this Operator}. <br />The main portal is the only underground opening associated with this Permit. The <br />Topaz decline was in development when work was suspended in October 1998. The decline <br />did not reach the underground orebody prior to the suspension of work. <br />"Secondary" Road: <br />Explain the "secondary" road to the upper bench and the purpose of the upper bench. <br />Response: <br />The upper bench is a water tank pad. It was constructed with a road and adequate <br />room to allow a delivery truck to access and fill the tank (the tank is buried in the slope below <br />the bench). <br />4 <br />
