December 19, 2001 Inspection by Bob Oswald: <br />No Permit Identification Sien or Permit Boundary Markers: <br />The lack of a Permit identification sign and Permit boundary markers is noted as a <br />"problem" and a Corrective Action was specified. <br />Response: <br />In January 2002 the Operator installed a correct Permit identification sign and <br />provided photographic evidence. <br />The Operator submitted a Proposed Affected Area boundary with the Annual Report <br />on January 28, 2003. Upon reaching concurrence with the Division on the Affected Area <br />boundary, the corners will be marked in the field. <br />Additional Buildings on Site: <br />A semi trailer and a wooden shed on skids are present but not shown on the map <br />provided by the Operator. <br />Response: <br />The 40' trailer is a portable dry facility; its location is shown on the updated site map <br />submitted with the last Annual Report. The portable metal building on skids has been <br />removed. <br />Unsecured Oil Storage: <br />The oil storage area is unsecured and uncontained. A Corrective Action was specified <br />to address the matter of leaning barrels and inadequate hydrocarbon management. <br />Response: <br />The Operator removed the oil barrels in January 2002 and provided photographic <br />evidence of the completion of this task. The site was revisited a number of times in 2002, and <br />there is no evidence of spilled oil or contaminated soil where the barrels were sitting. <br />Portal Unsecured: <br />The portal gate was grated but the grate was cut. The inspector recommended that the <br />grate be repaired. <br />Response: <br />The grate was repaired in January 2002. Subsequently, in November 2002, timber <br />was placed in front of the portal, and dirt was piled against the timber to prevent entrance to <br />the portal. <br />Out of Date Site Map: <br />Following the June 2, 1998 letter from the Division, which noted a number of items <br />that needed to be revised and updated on the site map, the Operator failed to provide a current <br />and accurate site map. As a result of the Operator's failure to respond to the Division's <br />inquiries, the lack of a current and accurate map is classified as a "problem" and a Corrective <br />Action was specified. <br />Response: <br />The Operator submitted an updated and correct site map with the Annual Report on <br />January 28, 2003. <br />3 <br />
