(Page 4) <br />MINE ID # OR PROSPECTING ID #: M-~nnn_nRR <br />INSPECTION DATE: 9/~ 5/(15 <br />INSPECTORS INITIALS: KAP <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce theirseverity when considered <br />by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />3. 11/ 30/ OS <br />During the inspection, it was discovered that excavation at the site is resulting in a vertical <br />highwall, whereas the approved mining plan states that material will be removed in a <br />manner resulting in a slope of 3H:1 V or less. This is a problem related to 34-32.51 16 of the <br />Colorado Revised Statutes, commonly referred to as the Colorado Land Reclamation Act <br />for the Extraction of Construction Materials for failure to perform the prescribed <br />PB or PV: 36_ <br />TOPIC(S): ~Cz <br />Corrective Action Within sixty 1601 days of receipt of this report, submit proof to the Division <br />that the existing highwall at the site has been graded to a slope of 3:1 or less, or submit a <br />Technical Revision to the Division for review stating that the highwall will possess a steeper <br />slope during mining and will be graded to a slope of 3:1 or less during the reclamation <br />process. <br />4• 11 / 30/05 <br />During the inspection, it was discovered that there is no stockpile of topsoil at the site, <br />which would be used for reclamation of the site. This is a problem related to 3.1.9 (1) of <br />the Construction Materials Rules and Regulations, for failure to remove and segregate <br />topsoil from other spoil and/or a problem related to 34-32.51 16 of the Colorado Revised <br />Statutes, commonly referred to as the Colorado Land Reclamation Act for the Extraction of <br />Construction Materials for failure to perform the prescribed reclamation plan. <br />PB or PV: ~B_ <br />TOPIC(S): ~_ <br />
