The problems should be corrected by the dates given, or they will become possible violations. The <br />possible violations should be corrected by the dates given to reduce their severity when considered by the Mined <br />Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions <br />taken. <br />PROBLEMS/POSSIBLE VIOLATIONS and CORRECTIVE ACTIONS <br />~~ <br />PROBLEM or POSSIBLE VIOLATION: PB CORRECTION DATE: October 23, 2006 <br />TOPIC(S): HB <br />DESCRIPTION: Potential exposure of ground water has occurred in the wash-out pit located north of the concrete <br />batch plant. Rule 3.1.6 (1) states that disturbances to the prevailing hydrologic balance of the affected land and of <br />the surrounding area and to the quantity or quality of water in the surface and ground water systems shall be <br />minimized both during and after mining. <br />CORRECTIVE ACTION NARRATIVE: Complete one of the following actions on or before October 23, 2006: <br />1. Backfill the existing wash-out pit to an elevation that is two (2) feet above the ground water table and provide <br />photographic documentation of completion, or <br />2. Demonstrate to the satisfaction of the Division that the water in the it is not round water. <br />PROBLEM or POSSIBLE VIOLATION: PB CORRECTION DATE: November 21, 2006 <br />TOPIC(S): TS <br />DESCRIPTION: The amount of salvaged topsoil on site is insufficient to reclaim the site in accordance with the <br />approved reclamation plan. The approved reclamation plan indicates that four (4) to eight (13) inches of topsoil will <br />be placed on the site during reclamation. Assuming an average depth of six (6) inches, the Division estimates that a <br />total of 18,550 cubic yards of topsoil will be needed to reclaim the disturbed area. The amount of topsoil on site and <br />available for reclamation is estimated to be approximately 6,000 cubic yards. Therefore, 12,550 cubic yards of <br />topsoil will need to be imported in order to complete the reclamation plan. <br />CORRECTIVE ACTION NARRATIVE: Complete one of the following actions within sixty days of receipt of this <br />report (estimated to be November 21, 2006): <br />1. Import at least 12,550 cubic yards of topsoil to the site in order to maintain at least 18,550 cubic yards of <br />topsoil available for reclamation, or <br />2. Submit a complete application for a permit amendment to annex the topsoil stockpiles at the south end of <br />the site and include them within the permit boundary for the Thompson Resource permit (operator must <br />
