The pond detail does nr,t depict the <br />two inflow points. The vent tan located above the office is also not <br />depicted on the map and the map and plans are nr~t ~;ertified. It may <br />be that the information is not current however our files do not <br />refle~.t more current information. <br />Field Inspection: <br />The inspection commenced at the north corner of the facilities area <br />near the office. The mine identificati~~n sign is located at the <br />intersection of the permit boundary and ~=ounty F:oad '~'3. It depicts <br />the mine address, telephone number, lease numbers, and various permit <br />numbers. The Colorado permit number listed r.n the sign is 7'~-55. "fills <br />is the interim program permit number. The ~=urrent permit number <br />reflected in various documents prepared by the Division is C-~~11-B1. <br />Therefore, TDN 87-~~~-107-1101? was issued tc. the Division for the <br />lacF: of a sign depicting the number cif the current permit authorizing <br />mining activities in accnrdan~:e with the Coloradr_ F,eq_ulations at F:ule <br />4.02.. <br />The inspection continued with observations of the sediment control <br />system. The perimeter ditch was inspected for structural integrity <br />along its length on the southwest side of the surface facilities. I <br />walF;ed the ditch to its inflow point at the sediment pond. Although <br />the ditch appeared to be intact it is overgrown with annual weeds. <br />The inspection of the sediment pond indicated that it has apparently <br />been quite same time since maintenance of the structure has been <br />undertaken. Like the perimeter ditch the pond is also overgrown with <br />a thicl! stand of Cattails. The west ban4: of the pond is in need of <br />maintenance. There are approximately five inflow pints an this ban4:. <br />These inflow points are in addition to the two designated inflr_,ws. <br />They are the result of erosion from concentrated sheet flow from the <br />facilities area. The northern mast paint is approximately 36 inches <br />deep, 36 inches wide at the top, 1'2 inches wide 3 inches from the <br />bottom and narrowing to the 3 inches in width at the bottom. The <br />remaining infl~~w points are substantially smaller. In addition, the <br />Division indicated in its 6/16/87 inspection report that although the <br />sediment control system appears to be functional, routine maintenance <br />of all the strutures is required. The report further states that the <br />maintenance should be performed as soon as practicable. The August <br />inspection report also indicates that maintenance of the sediment <br />control system is recommended. In any event, TDN 87-~72-107-11(21 was <br />issued for failure to maintain the sediment pond to prevent short <br />circuiting to the extent possible and failure to prote~.t the side <br />slopes of the sediment pond against erosion. The regulations cited in <br />the TDN are 4.05.6(61 and 4.05.'3(3).The inspectir_~n of the pond alsr_~ <br />included observations of bath the principal and emergency spillways. <br />Each was found to be inta~.t although the upstream end of tFie <br />emergency spillway is beginning to degrade. The spillway is <br />constructed of cinder blocks. The outside row of cinder bloc=ks is <br />beginning to seperate from the rest of the structure.
