Sorenson) was contacted to advise of these conditions. <br />4 Mar. '97 DMG (A. Sorenson) letter to COM, loc. (C. Bernhardt) requiring reinstallation <br />of the approved spray evaporation system ready for inspection by April I, 1997. <br />(This deadline was not met). <br />March '97 Approximately three days after the above events, COMB (R. Mason) was <br />contacted by Mr. David Tipton of Franklin Mill to advise of his intent to conduct <br />a site visit at the Gold Hill Mill and remedy any deficiencies found. in our <br />telecon, 1 advised Mr. Tipton of my understanding of permit requirements and <br />existing conditions at the millsite, and 6e agreed that remedial actions were <br />required. <br />I Apr. '97 On or about this date 1 was contacted dvectly by Mr. "fipton for the purpose of <br />accomplishing a joint survey of millsite conditions and remedying an elecaica! ., <br />problem which had rendered the primary pump at the Hazel A inoperable. We <br />were able to fix the p~rablem and function check the pump. Mr. Tipton and I <br />agreed to work together proactively to assure continued compliance of the millsite <br />with permit requirements. <br />9 Apr. '97 DMG (G. Curtiss) h~spection Report ide~fying 1 PV: failure to complete <br />rcimtallation of the spray evaporation system by tlx esTSblished deadline. Hearing <br />scheduled for June 25 -26 to determine violation of permit conditions. <br />26 Apr. '97 DM(i (A. Sorenson) letter to COM, lnc. (C. Bernhardt) requesting required <br />ground water monitoring results far the preceding four (4) quarters and <br />establishing parameters and detection limits for dissolved metals in sampled water. <br />Letter farther regnires operation of spray evaporation system a minimum of <br />eight (8) boars per day anti! tailings pond has been aabataatitally emptied of <br />standing wrater. <br />10 May '97 DMG (A. Sorenson) inspection report identities 6 PB including inadequate <br />spcinYler heads on spray evaporation system, damaged animal exclasioa <br />fence around tailiaga pond, and other items: correction deadline of May I6, <br />1997. Report stipulates that Hazel A water level should be kept as low as `- <br />posstble, but in no event should freeboard be less than twelve inches, else a <br />possible violation. Site is to be monitorrQ on a daily basis until further notice. <br />23 May '97 DMG (M. B. long) letter to COM, lnc. (C. Bernhardt) transmitting a formal <br />NOV relating to faihn-e to ea7ablich the required spray evaporation system by the <br />established deadline ant setting a formal public hearing for June 25 - 26, 1997. <br /> <br />
