Long) letter to Fraser transmitting Board Orders of same date: COM, Inc. <br />ordered to Finalize repairs to the tailing pond Liner by November 20, 1995; COM, Lnc. <br />ordered to remove all mill tailing fmm the Hazel-A adit and place the tailing removed <br />into the Gotd Hill Mill tailing impoundment by December 10, 1995. <br />30 Nov. '95 DMG (A. Sorenson} Inspection Report identities 1 PB and 1 PV; liner damage noted. <br />5 Dec. '95 DMG (A. Sorenson) laspection Report identifies 1 PV; full liner integrity still not <br />established. Operator required to survey adit and report the storage capacity to the <br />Division. Four bare-holes were observed in adit sill. Operator was directed tD seal <br />these bore-holes, <br />11. Dec. '95 COM1r (R. Mason) particigates in DMG (A. Soreasoa) inspection of millsite. Results, <br />1 PV, include observation that pond liner main repairs sad lining of dam raise are <br />complete, but holes still exist near water line on upstream face of retaining dam. <br />Tailings have been very substantially, but in COME opinion oat completely, removed <br />fmtn the Hazel A. DMG opinion is that Hazel cleanup is essentially complete. <br />14 Dec. '95 COM, lne. submits repot on compliance itetns to MLRB acting upgrades to the Hazel <br />A adit, the tailings pond, diversion ditches and berms, the aedimeat dam, removal of <br />oii~ontsminated soil, submission of a stvrmwater noanagemem plan, trash cleanup, and <br />progress on a revised water management pled; preliminary calculations show a <br />water storage capacity behind the bulldtead of about 33.000 gallons (4,400 cu. 8. ). <br />The repoR further detailed plans and actions relating to the cleanout of tailings slimes (~ <br />'fault gouge' as claimed earlier by COM, lac.), and accompHshmems and difficulties <br />related to repair of leaks in the tailings pond liner. <br />Note: The 4,400 cu. ft, r+eportod above represents only shoat 4196 of the roughly <br />!0,1300 gallon estimate given by DMG in the Memo to File dated 25 Jane '95, la <br />addition, exhibit A, item 3reports a re-routing of the diversion ditch around the <br />south end of the tailings pond so it no Longer delivers runoff to the pond. This <br />will affect the calculation of storm storage requirements. <br />14 Dec. '95 MLRD Stat2 makes presentation to the MLRB recommending issuance of a NOV and <br />CdtDO for failure to complete satisfactory tailings pond Inver repairs by the specitted date <br />(November 1, 1995), and noting possible civil penalties of S50 to S2W/day for violation <br />(43 days of violation as of this hearing date).
