Mat. Division (S. Laudeman) ltr to COM, tnc. ('l'. Hartley) requiring <br />documentation of the removal of wastes from the millsite; such documentation to <br />include a narrative description of waste removal to a permitted landfill, <br />characterization, including lab analyses, of oil contaminated soils, sad manifests <br />documenting waste transpott_ Haz. Mat. Division to be nodhed at least three days <br />prior to waste removal; representadve of Haz. Mat. Div., DMG, BCHD, or Town of <br />Gold Hill to be present at removal. (We have not been able to establish that any such <br />notice was provided to any of the named agencies prior to the removal which has ahrady <br />oaurred.) <br />25 Sep. '95 DMG (A. Sorenson) lnspecdon Report indicating pumping and junk removal in progress. <br />27 Sep. '95 DMG (A. Sorenson) br to Dufford and Brown, P.C. (R. l+aayo) subject: bond reduction <br />request. COME follow up required within compliance review period, probably to oppose <br />any such reduction .at this time (i.e. pending full and complete compliance with all <br />outstanding Board/Staff direction). We Hoed to be satisfied that all legally possible <br />environmental remedies Gave been folly underwritten by the bond level established. <br />9 Oct. '9~ inspectioa of Hazel A and general millsite today shows partial earth bacl~ill of entrance <br />to Hazri A around a piece of used culvert, with a lockable steel door. Water level <br />behind bulYhead shows about ibur feet ttreboard, about two to two and 1 /2 feet of water <br />and solids. Bulkhead is leaking some; moisture is showing is backfilled earth outside <br />edit. Deadline for required lined coffer dam and pump-back system appears not <br />to have been met. Action: Call DMG to identify situation. <br />l~~ Oct. '95 DMG (A. Sorenson) ltr to COM, loc. (G. Fraser) indicating that DMG analysis of Haul <br />A slimes shows these to be from the Gold Hill Mill and requiting their cleanup NLT 1 <br />Dec. '95. <br />11 Oct. '95 DMG (A. Sotmson) inspection report shows no problems/possible violations. Cotner <br />dam and pump-back system now in place and operational, although deadline was not met. <br />Cited corrective actions/follow-on work include: reseeding of disturbed area around <br />diversion ditches (due end of November '95), emplacement of bay bales below outlet of <br />settling basin (ao deadline), completion of liner repairs and resetdng of game fence (due <br />1 November '95), completion of minimum fifteen (15) foot horizontal backfill around the <br />CMP (36' culvert) - (no deadline), t~emoval of contaminated soil from <br />compressor/vacuum pomp (due l Nov. '95). The Haiardous Materials and Waste <br />Management L?ivisioa has lead responsibility far assuring that the contaminated soil is <br />property desposed of once it leaves the sire. <br />
