RECORDS REVIEW <br />Certificate of insurance: Policy number GLM 10172 has been <br />issued to NCIG Financial, Inc. for an aggregate amount of <br />$1,000,000.00. The expiration date of the current policy is <br />12/22/92. <br />Ownership and control: The information contained in the permit <br />application was compared to that in the Applicant Violator System <br />and was found to be the same. <br />Bond: The bond consist of 3 certificates of deposit totaling <br />$288,000.00. Two of the certificates are automatically renewable <br />but certificate number A39508 is not. MLRD has an agreement with <br />the issuing bank that they will be notified at least 30 days in <br />advance if the certificate will not be renewed. Colorado rules <br />require certificates of deposit be automatically renewable <br />(3.02.4). The amount of bond appears to be adequate. <br />Blasting: No blasting at this time. <br />NPDES or discharge permit: The CDPS permits for the mine site <br />and the loadout both expire on June 30, 1992, No discharges were <br />occurring. <br />water monitoring: The 1991 annual water monitoring report was <br />submitted with all the required information. <br />Air quality monitoring: There are several air emission permits <br />covering different areas of the mine operations. Most of these <br />have an expiration date of 8/3/93. <br />SPCC Plan: No petroleum products are being stored on the mine at <br />this time. <br />Pond Inspections: The quarterly inspections of ponds were done <br />and submitted for 1991. The quarterly inspection for the first <br />quarter o£ 1992 was not at or near the mine although MLRD did <br />
