It is expected that the Project Safety Plan will be comprehensive and sufficiently detailed so that <br /> every member of the work crew, including sub-contractors, on site understands their <br /> responsibilities and the responsibilities of CONTRACTOR to ensure a safe and injury free work <br /> place. The Project Safety Plan must be provided to the work crew members in the language in <br /> which they are most readily able to comprehend. <br /> Upon receipt of the SHEAP, the OWNER will either accept the plan, or request that more detail <br /> be provided. <br /> The CONTRACTOR and their employees or sub-contractors are expected to follow the SHEAP <br /> during performance of every component of the Project. If the PROJECT MANAGER determines <br /> that the SHEAP is not being followed, either in whole or in part, the Project may be immediately <br /> suspended, without stopping the mob clock, until such time as the SHEAP is implemented. <br /> The following items must be addressed in the SHEAP: <br /> 1. Personal protective equipment (hard hats, steel toe boots, gloves, safety glasses, etc.) <br /> must be worn at all times and be appropriate for the tasks being performed; <br /> 2. First aid kits must be provided and maintained for the duration of the project; <br /> 3. Accident prevention and safety when operating motor vehicles and equipment; <br /> 4. Fall protection & prevention for all work conducted where fall hazards exist; <br /> 5. Overhead physical and electrical hazards; <br /> 6. Hot work safety procedures and fire prevention; <br /> 7. Welding fumes, exhaust, and vapor protection; <br /> 8. Prevention and clean-up of environmental contamination; <br /> 9. Alcohol and drugs are not permitted on the job site; <br /> 10. Accident reporting; <br /> 11. Daily safety meetings; <br /> CONTRACTOR shall provide a signature block similar to the one below that each employee <br /> can sign to verify that they have read and understand the safety aspects of the project, <br /> and accept the provisions of the SHEAP. <br /> Contractor Representative: <br /> Signature Date <br /> Employee: <br /> Signature Date <br /> Measurement and Payment <br /> There will be no measurement for payment for this task. Payment shall be for the lump sum bid <br /> price under Item 2.0 of the Bid Schedule. This lump sum bid will cover all costs for developing <br /> and implementing the SHEAP, including writing and revising the plan as required, provisions and <br /> maintenance of safety equipment and personnel, conduct of daily safety meetings, employee <br /> training, and all other items necessary and incidental to fulfilling the requirements of this task and <br /> of the SHEAP. Payment for this task will only be made on the final invoice (before retainage) <br /> following completion of all work and upon approval from the PROJECT MANAGER. <br /> Page 7 of 9 <br />
