Operation of heavy equipment in excavation/earthmoving or demolition activities <br /> presents potential physical hazards to personnel. The following PPE shall be worn <br /> whenever such equipment is present: steel-toed shoes, safety glasses or goggles, high <br /> visibility (orange) safety vests, and hard hats. Personnel should at all times be aware of <br /> the location and operation of heavy equipment, and take precautions to avoid getting in <br /> 5 <br /> the way of their operation. Personnel walking or driving in the vicinity of heavy <br /> equipment shall make eye contact with the heavy equipment operators before <br /> approaching or traveling near heavy equipment. Personnel driving vehicles in areas <br /> where there is heavy equipment shall ensure their headlights are on. While operating <br /> machinery seat belts will be worn. Personnel working near heavy equipment shall not <br /> turn their back to the equipment and shall keep the equipment in site at all times. <br /> Personnel shall not pass under an elevated boom or attachment. If personnel need to <br /> approach heavy equipment (e.g., to speak to the equipment operator), hydraulic <br /> attachments shall be placed in a"zero energy" state. <br /> All heavy equipment on-site will be thoroughly washed prior to being delivered to the <br /> site to avoid spreading of weeds. <br /> Trenching/Excavation <br /> Trenches and excavations may pose a physical hazard to site personnel. All trenching and <br /> excavation work shall comply with the requirements of 29 CFR 1926, Subpart P. Some <br /> requirements for safe trenching are: <br /> • Workers will not go into trenches or excavations. If for some reason, worker access is <br /> necessary, the following actions shall be implemented: <br /> • A Competent Person will assess the excavation for entry. <br /> • If employees are required to be in trenches 4 feet deep or more, an adequate <br /> means of exit, such as a ladder or steps, will be provided and located no more <br /> than 25 feet from any work area. <br /> • The excavation shall be assessed for oxygen deficiency or hazardous gaseous <br /> conditions are possible, air in the excavation or trench shall be tested. Controls <br /> shall be established to assure acceptable atmospheric conditions. When <br /> flammable gases are present, adequate ventilation shall be provided or sources <br /> of ignition shall be eliminated. Attended emergency rescue equipment, such as <br /> breathing apparatus, a safety harness and line, basket stretcher, etc., should be <br /> easily available where adverse atmospheric conditions may exist or develop in <br /> an excavation or trench. A log of all test results shall be maintained. <br /> • Any and all excavations and/or trenches which exceed 5 feet in depth must be sloped, <br /> shored, sheered, braced or otherwise supported. Sloping angles and or shoring/bracing <br /> • Requirements shall be determined after an inspection of the soils and conditions by a <br /> competent individual.
