PMYOPIV <br /> • Pinyon project number and task number <br /> • Project name and location (include state in which site is located to ensure that laboratory uses state- <br /> approved analytical methods, especially when shipping samples to a non-local lab). <br /> • Sampler's initials or signature. <br /> • Pinyon sample identification number (a unique number for each sample). <br /> • Date (mm/dd/yy) and time sample collected (always designate time in military units). <br /> • Sample matrix: Soil, Groundwater, or Air Summa Canister. Some COCs may classify matrix type as Solid, <br /> Liquid, or Air. <br /> • Location or other unique identification of sample (keep short). <br /> • Type and number of containers provided for with each sample ID (e.g.,four 4-oz.glass jars). <br /> • Preservation (if required) (e.g., HCI, NaOH, ice, etc.) <br /> • Required analyses, including method number. When filling out the analysis to be completed for each <br /> sample, provide specific methods not just the sample parameter, e.g., "TSS, method 2540D" instead of <br /> "TSS". Typically, this information should be provided in the Scope of Work. It is always good practice to <br /> double check that the COC matches the Scope of Work. <br /> • Check box for each sample to be analyzed using the corresponding method. <br /> • Remarks about the sample, such as color, viscosity, temperature, clarity, etc. <br /> • Requested turn-around time for analyses. <br /> • Other pertinent sample instructions can be included (e.g., "hold sample for additional analysis" or"results <br /> to be faxed", etc.) <br /> • Signature, date and time of exchange for each transfer of samples. This must be acknowledged by both <br /> parties involved in the transaction. <br /> Once the COC is complete, double check that each sample ID description has a date sampled, time sampled, <br /> number of containers, preservative type, matrix type, and analysis method provided. <br /> Chain of Custody Documentation <br /> "Relinquished by" and "Received by" fields are provided on the COC to document the chain of custody, <br /> SOMEONE must be responsible for the samples at all times. The person relinquishing the samples must <br /> provide a signature, date, and time of exchange in the appropriate fields and the recipient must do so as well <br /> in the adjacent box. The times provided be each party should match closely, within several minutes of the <br /> other. This procedure applies every time the samples change hands, even between Pinyon employees.
