PMYOPIV <br /> Do not leave the samples out of sight in an unsecured (unlocked) area as this this violates the custody chain. <br /> General Guidelines for COCs <br /> • Every box or empty line should be filled out. If no answer is to be provided,write "none" or"NA". <br /> • Every line should be filled out for each individual sample. It is not acceptable to write the answer for the <br /> first row and either draw arrows to indicate the same answer for subsequent samples or write"see above". <br /> • Empty rows that remain after all samples have been identified should be crossed out by the author and <br /> initialed. <br /> • Write as legibly and neatly as possible to ensure that lab personnel can read the COC. <br /> • When filling out the analysis to be completed for each sample, provide specific methods not just the sample <br /> parameter, e.g., "TSS, method 2540D" instead of"TSS". <br /> • The order of samples provided on the COC should be the order the samples will appear in the report, <br /> not necessarily in the order they were sampled. This streamlines the reporting process. <br /> • Pen should always be used and forms should be filled out on a solid surface with sufficient pressure to <br /> ensure that writing transfers over to the carbon copy. If a mistake is made, it should be crossed out with <br /> a single strike-through line (not scribbled out) and initialed by the author. As a standard rule of thumb, if <br /> a COC contains three crossed out mistakes it may be prudent to start over on a clean form. <br /> • A copy of the completed COC that has been signed by the lab should always be saved, either as a hard <br /> copy or an electronic copy. If using a tablet field form, a photograph of the COC should be included with <br /> form submittal. There should be an appropriate field where a picture of the COC can be uploaded.
