Proper labeling and documentation of samples for laboratory analysis is required under RCRA- <br /> defined protocols. Chain-of-custody seals and forms provide a guarantee that samples collected in the field are <br /> not subject to tampering or improper storage conditions. The chain of custody is a record of all personnel <br /> who retained custody of the samples at any point from the time they were collected until they reach the <br /> laboratory. <br /> A copy of a sample chain-of-custody form, sample label and custody seal are attached in Appendix D of the <br /> Vista Del Rio Soil Sampling and Analysis Plan. The chain of custody form is filled out and submitted with the <br /> samples whether they are shipped or dropped off in person. Each sample must have a label with its unique <br /> sample ID. Custody seals are used on individual samples or on a cooler containing samples to show evidence <br /> of sample tampering. Labels, chain-of-custody records and custody seals can be provided by the <br /> laboratory. Each laboratory will have their own unique form, or it is acceptable to use the standardized Pinyon <br /> form. <br /> 2.0 Procedure <br /> At a minimum, the information below should be recorded on the form (the following explanation is keyed to <br /> the attached sample form): <br /> • Client (Pinyon) <br /> • Address (3222 S Vance St., Suite 200, Lakewood CO 80227) <br /> • Telephone number (303-980-5200). <br /> • Project manager and email contact information. <br /> Chain of Custody Form <br /> December 23,2022 <br /> Page I of 3 <br />
