Process water discharges from operations that produce the following commodities (SIC Code in parentheses): <br /> • Gypsum (1499); • Sodium sulfate(1474); <br /> • Asphaltic minerals(1499); • Frasch sulfur(1479); <br /> • Asbestos and wollastonite(1499); • Bentonite(1459); <br /> • Barite(1479); • Magnesite(1459); <br /> • Fluorspar(1479); • Diatomite (1499); <br /> • Salines from brine lakes(2899); • Jade(1499); <br /> • Borox(1474); • Novaculite(1499); and <br /> • Potash (1474); • Tripoli(1499) <br /> • Phosphate Rock(1475); <br /> 3. Chemical addition <br /> Discharges with chemical addition (including, but not limited to chemical additions at any point in the treatment process, <br /> release agents,etc),are eligible for coverage under this permit only if the Division approves the use of the specific chemical(s) <br /> and provides notification of such approval to the permittee. <br /> To request Division approval,the permit applicant must submit a list of proposed chemicals, including dosage rates, used in the <br /> treatment process. Additionally,the applicant must submit an MSDS for each chemical proposed for use. In granting the use of <br /> such chemicals,the Division may impose additional limitations and monitoring requirements in the permit certification. <br /> Chemicals used in waters that will or may be discharged to waters of the State must be used in accordance with all state and <br /> federal regulations,and in strict accordance with the manufacturer's site-specific instructions. <br /> 4. Obtaining and maintaining Authorization under this permit <br /> a. Application Requirements: To obtain authorization for discharges under this permit: <br /> i. The applicant must meet the eligibility requirements under Parts I.A.1. <br /> ii. For stormwater discharges,the applicant must develop a Stormwater Management Plan (SWMP) in accordance <br /> with the requirements of this permit prior to submitting an application to the Division,and must certify in the <br /> application that a SWMP has been completed. <br /> iii. The applicant must submit a complete,accurate,and signed permit application,on a form provided by the <br /> Division, by mail or hand delivery to the Division at least 60 days before the anticipated date of discharge; or for <br /> stormwater-only discharges,at least 60 days before the facility commences industrial activity that may result in a <br /> discharge of stormwater. The applicant must sign the application in accordance with the requirements of Part I.F <br /> (Reporting and Recordkeeping)of this permit. The complete application shall be submitted to: <br /> Colorado Department of Public Health and Environment <br /> Water Quality Control Division <br /> Permits Section,WQCD-PCP-B2 <br /> 4300 Cherry Creek Drive South <br /> Denver,CO 80246-1530 <br /> iv. The applicant(s) must receive written notification that the Division granted permit coverage. <br /> b. Permit Certification Procedures: Following review of the application or other information,the Division may: <br /> i. request such additional information as is reasonably necessary to evaluate the discharge; <br /> ii. delay the authorization to discharge pending further review; <br /> iii. notify the applicant that additional terms and conditions are necessary; <br /> iv. provide a compliance schedule in the certification for terms and conditions that are new or more stringent than <br /> previous conditions; <br /> v. deny the authorization to discharge under this general permit. <br />
