PART <br /> Page 8 of 67 <br /> Permit No.: COG500000 <br /> The Division will notify the applicant in writing of its request or determination for items i.—v. <br /> c. Alternative permits <br /> i. Division required alternate permit coverage:The Division may require an applicant or permittee to apply for an <br /> individual permit or an alternative general permit if it determines the discharge does not fall under the scope of <br /> this general permit. In this case,the Division will notify the applicant or permittee that an individual or alternate <br /> permit application is required. <br /> ii. Permittee request for alternate permit coverage: A permittee authorized to discharge under this general permit <br /> may request to be excluded from coverage by applying for an individual permit. In this case,the permittee must <br /> submit an individual application,with reasons supporting the request,to the Division at least 180 days prior to any <br /> discharge. The permittee's authorization to discharge under this general permit is terminated on the effective <br /> date of the individual permit. <br /> d. Permit Expiration,and Continuation <br /> A permittee desiring continued coverage under this general permit must reapply at least 180 days in advance of the permit <br /> expiration date. The Division will determine if the permittee may continue to discharge under the terms of the general <br /> permit. An individual permit may be required for any facility not reauthorized to discharge under the reissued general <br /> permit. <br /> If this permit is not reissued or replaced prior to the expiration date, it will be administratively continued and remain in <br /> force and effect. For permittees that have applied for continued permit coverage,discharges authorized under this permit <br /> prior to the expiration date will automatically remain covered by this permit until the earliest of: <br /> i. An authorization to discharge under a reissued permit,or a replacement of this permit,following the timely and <br /> appropriate submittal of a complete application requesting authorization to discharge under the new permit and <br /> compliance with the requirements of the new permit; or <br /> ii. The issuance and effect of a termination issued by the Division;or <br /> iii. The issuance or denial of an individual permit for the facility's discharges; or <br /> iv. A formal permit decision by the Division not to reissue this general permit,at which time the Division will identify a <br /> reasonable time period for covered dischargers to seek coverage under an alternative general permit or an <br /> individual permit. Coverage under this permit will cease when coverage under another permit is <br /> granted/authorized; or <br /> v. The Division has informed the permittee that discharges previously authorized under this permit are no longer <br /> covered under this permit. <br /> 5. Transfer of permit coverage <br /> A permittee may transfer coverage under this general permit to a new discharger if all of the following conditions are met: <br /> a. The permittee (existing discharger)and new discharger submit a complete and accurate Notice of Transfer form, signed <br /> by the permittee and the new legal entity,to the Division at the address listed in Part I.A.4,at least 30 days prior to the <br /> proposed transfer date. The Notice of Transfer form must contain a specific date for transfer of permit responsibility, <br /> coverage,and liability. <br /> b. The type of industrial activities and practices remain substantially unchanged. <br /> c. The Division does not notify the permittee of the need to submit a new application for coverage under the general <br /> permit or for an individual permit. <br /> d. The Division does not notify the permittee and new discharger of its intent to revoke coverage under the general <br /> permit. <br /> 6. Modifying an existing permit <br /> A permittee may modify an existing permit certification if all of the conditions identified below are met. <br />
