STATE of COLORADO HIGHWAY ACCESS PERMIT <br /> ADDITIONAL TERNIS and CONDITIONS <br /> December 17, 2021 PERMIT No. 321125 <br /> Permittee(s): Gary Knippa <br /> Location: Lake County on CO Highway 024A, near Mile Ref. Pt. 178.141 Right <br /> ii) A TCP that shall provide accessibility features to accommodate alt pedestrians including <br /> persons with disabilities for all pathways during construction. <br /> 10.The horizontal axis of the access to the State Highway shall be constructed perpendicular to the <br /> centerline of the highway and extend from the edge of the roadway a minimum distance of 40 feet, <br /> or to the property line, whichever is greater. This design shall be in conformance with section 4 of <br /> the Access Code. <br /> 11. Side slopes shalt be at a 4:1 stope on the roadway. The roadway shatl slope away from the highway <br /> at a -2% grade for the first 20 feet of driveway. This design shall be in conformance with section <br /> 4 of the Access Code. <br /> 12. Access width and geometry shatl be determined by designing the access for the largest vehicle <br /> using the access on a consistent basis per Section 4.5 of the Access Code. <br /> 13. Immediately upon completion of earthwork the access shall be hard-surfaced a minimum distance <br /> of 50 feet from the traveled way, or to the CDOT Right-of-Way, whichever is greater. Where the <br /> hard surface is to abut the existing pavement, the existing pavement shatl be saw cut and removed <br /> a minimum of one foot back from the existing edge, or until an acceptable existing cross slope is <br /> achieved. Surfacing shall meet CDOT's specifications with minimum surfacing to be equal to, or <br /> greater than, existing highway conditions in conformance with section 4 of the Access Code. <br /> 14.Materials, Placin�, and Compaction <br /> Unless the Applicant has approval from the Access Manager who may state otherwise, the <br /> following are minimum requirements for driveway construction: <br /> Hot Mix Asphatt Option (HMA) <br /> Base: 16 inches of class 6 gravet with maximum 6-inch lifts; <br /> Surface: 4 inches of HMA in two, 2-inch lifts; <br /> Compaction of the subgrade, embankments and backfill shall comply with sections 203 <br /> tt 304 of the Colorado Highway Standard Specifications for Road and Bridge <br /> Construction. <br /> Concrete Pavement Option: Porttand Cement f PCCP) <br /> Base: 4 inches of ctass 6 gravet; <br /> Surface: A minimum of 6" of doweled and tied PCCP. <br /> Compaction of the subgrade, embankments and backfill shalt comply with sections 203 <br /> �t 304 of the Colorado Highway Standard Specifications for Road and Bridge <br /> Construction. <br /> 15.This permit altows for onsite construction as long as such use does not violate any terms of the <br /> permit. Permittee shall coordinate with CDOT for onsite construction and shall provide a traffic <br /> control plan and proof of liability insurance. If the access location, volume, or turning movement <br /> for onsite construction is different from the permitted access, a new temporary construction permit <br /> may be required. <br />
