STATE of COLORADO HIGHWAY ACCESS PERMIT <br /> ADDITIONAL TERMS and CONDITIONS <br /> December 17, 2021 PERMIT No. 321125 <br /> Permittee(s): Gary Knippa <br /> Location: Lake County on CO Highway 024A, near Mile Ref. Pt. 178.141 Right <br /> 1. This permitted access is only for the use and purpose stated in the Apptication and Permit. This <br /> Permit is issued in accordance with the State Highway Access Code (2 CCR 601-1), hereafter <br /> referred to as the "Access Code", and is based in part upon the information submitted by the <br /> Permittee. Any subsequent relocation, reconstruction, modifications, the type of traffic using the <br /> access or 20%increase in volume to the access or shalt require a new application and coordination <br /> with Colorado Department of Transportation, hereafter referred to as"CDOT". Any changes causing <br /> non-compliance with the Access Code may render this permit void, requiring a new permit. <br /> 2. This permit replaces any and all additional access permits that may be in existence for this access. <br /> 3. This permit is for gravel pit use for parcel 263327�0009. <br /> 4. The traffic volume shall be 16 DHV(design hourty volume). There shall be no more than four trucks <br /> entering or four trucks exiting the access in a 60-minute period. <br /> 5. The Highway Access Category is NR-B. <br /> 6. This access shall have a full turning-movement. <br /> 7. Permittee shall install a STOP sign at the access. <br /> 8. This access shall be designed and constructed to CDOT's standards and may be required to include <br /> sidewalk or trail, curb and gutter. <br /> 9. A Notice to Proceed CDOT Form 1265 is required before beainnin�construction on the access <br /> o�anv activitv within the hi�hwav Ri�ht-of-Way.To receive the Notice to Proceed the Appticant <br /> shall submit a complete packet to CDOT (to Nick Nordquist - 970-683-6280, <br /> nicholas.nordquist@state.co.us)with the following items: <br /> (a) A cover tetter requesting a Notice to Proceed, and the intended date to begin construction. <br /> (b) One (1)etectronic copy plan set (11"x 17"with a minimum scate of 1"= 50')This plan set must <br /> be in full compliance with the State Highway Access Code. The plans shatl provide the contact <br /> information for the Design Engineer, if applicabte, and the following items: <br /> i) Plan view with driveway dimensions- turning radius, width, stope, gates, etc. <br /> ii) Typicat road section - existing and proposed sub base, base, pavement, and shoulder <br /> dimensions. <br /> iii) Centerline profile of the access/highway connection showing depths, driveway slope, etc. <br /> (c) Certificate of Insurance for Liability as per Section 2.3(11)(i)of the State Highway Access Code, <br /> naminQ CDOT on the face of the certificate as "an additional insured for Qeneral liabilitv"; <br /> (d) A certified Traffic Control Ptan (TCP) in accordance with Section 2.4(6)of the Access Code and <br /> the Manuat on Uniform Traffic Control Devices (MUTCD); <br /> i) A TCP that identifies the correct wording, number, spacing, and type of devices to be used, <br /> according to MUTCD standards and CDOT's Work Zone Safety Guidelines for Municipalities, <br /> Uti(ities, and Contractors, and be based on the highway speed, lane width, and location; <br /> and <br />
