Stormwater Management Plan <br /> j. Non-industrial waste sources <br /> Worker trash will be removed from the site once a week. Generated waste will be <br /> removed from the site and disposed in a County or State approved waste disposal <br /> facilities. <br /> k. Other areas or procedures <br /> Site personnel will provide notice to the Mine manager or his designated representative, <br /> immediately upon identification of a reportable spill. A spill report form is presented in <br /> Appendix-2. <br /> Petroleum products spills in excess of Reportable Quantities(RQs),will be reported by <br /> the Mine Manger or his designated representative,to the EPA National Response Center <br /> (1-800-424-8802),the Colorado Department of Public Health and Environment(877- <br /> 518-5608)and the Division of Reclamation, Mining and Safety(303-866-3567). <br /> INSPECTION AND MAINTENANCE <br /> 1. Inspection Schedules <br /> a. During initial site activities,water control measures will be inspected at least once every <br /> fourteen(14)calendar days, and within 24 hours after the end of a precipitation or <br /> snowmelt event that causes surface erosion, ECMs will be inspected monthly from April <br /> through November thereafter <br /> b. During snowy months(November through May), water control measures will be <br /> inspected and maintained as field conditions dictate. The second year after water control <br /> measures are in place and following the first winter event, ECMs will be inspected <br /> quarterly until the first snow fall returns. <br /> 2. Inspection Procedures <br /> • Verify and maintain the integrity of check dams(US EPA, 1992)(VDCR, 2005)and <br /> • Record site maintenance activities. <br /> a. Maintenance/Replacement and Failed ECMs <br /> Water control measures maintenance will be completed within 7 calendar days after failed ECMs <br /> have been identified. Sediment from erosion control structures will be removed, as necessary,and <br /> disposed in designated areas. <br /> b. Record Keeping and Documenting Inspections <br /> Inspections and maintenance activities(Appendix 4)will be documented, and records will be kept <br /> in the facilities office for a period of 3 years. <br /> 3. Agency Storm Water Inspections <br /> The Mine Manager, or his designated representative, must walk the site with the regulatory <br /> inspector and document any deficiencies or suggestions noted during the inspection. The Mine <br /> Manger shall prepare an Inspection Report. Deficiencies of any type, field or documentation- <br /> related,will be noted on the Inspection Report as a deficiency and resolved as soon as practicable <br /> or as directed. <br /> The Mine Manger or his designated representative will report to the agency inspection <br /> immediately,but no later than I-hour after the inspector has left the jobsite. All storm water or <br /> erosion and sediment(E&S)agency visits to the PENN MINE site,whether an official inspection <br /> occurred or not,will be reported to the Mine Manger or his designated representative.
