<br />Application for Permit Amendment (2000 Amendment) was submitted to the Colorado Division
<br />of Minerals and Geology on September 22, 20009 and approved on March 15, 2001, and the
<br />conditions of the terms of the amendment were transmitted on May 8, 2001.10 The United
<br />States Department of Agriculture, Forest Service (US Forest Service) issued its Environmental
<br />Assessment of the 2000 Amendment on June 21, 2001.11 The US Forest Service issued its
<br />Decision Notice and Findings of No Significant Impact of the 2000 Amendment onto the
<br />National Forest System Lands on August 30, 2001.12 Lastly, a request for Technical Revision to
<br />the Applicant’s Pikeview Quarry operation was submitted on July 24, 200113 and approved on
<br />August 31, 2001.14 This technical revision has been referred to as Technical Revision 11 (TR-
<br />11).
<br />3.1 Permit Application and Approval
<br />The Original Permit Application and Restated Permit Application contain information and data
<br />in Maps and Exhibits attached to the application pertaining to the design, aggregate extraction,
<br />and reclamation of Pikeview Quarry. These documents contain, in part, the results of
<br />engineering and geologic evaluations prepared by Leigh Whitehead & Associates, Consulting
<br />Engineers. An example of these evaluations is illustrated in the Mining Plan Map from the
<br />Original Permit Application shown in Figure 1. Another example of engineering evaluations is
<br />contained in Restated Permit Application, Exhibit E, Reclamation Plan, which contains
