• <br /> December 30, 2019 <br /> Lucas West <br /> Division of Reclamation, Mining& Safety <br /> 1313 Sherman St, Suite 215 <br /> Denver, CO 80203 <br /> Re: Permit Holder: Western Slope Flagstone Quarry#2 <br /> Permit No: M-1996-076 <br /> Application: Technical Revision/Updated Corrective Action Response <br /> Dear Mr. West: <br /> I am the operator of Western Slope Flagstone, LLC and holder of permit M-1996-076, <br /> which operations are situated in Section 35, T1OS, R98W, 6th PM in Mesa County Colorado. <br /> Pursuant to Rule 1.8.4, DRMS Mineral Rules and Regulations(August 2006), I submit this <br /> request for a Technical Revision to the permit as set forth herein. <br /> Western Slope Flagstone, LLC conducts mining operations in three parcels of land <br /> situated in Mesa County, and referred to as Assessor's Parcels 2709-344-00-070 and 2709-353- <br /> 00-067 and 2937-021-00-102, all owned as part of a trust("the Fontanari Trust"). <br /> Toward fulfillment of corrective actions recommended by our regional mining inspector I <br /> submit the following narratives. <br /> Permit Boundary Markers <br /> Many of the markers along the parcel boundaries serve well to identify permit <br /> boundaries, however to the uninitiated,they are difficult to see—especially during the summer <br /> months when the green metal post was surrounded by green field crops or other flora. <br /> Accordingly, and at the recommendation of our local inspector,we added boundary markers to <br /> those already standing and covered each of the metal posts with a white PVC sleeve in hopes of <br /> making the marker stand out from the surrounding vegetation.
