CCV EmergencA Response Plan <br /> 5.2.3 Communications Failure: <br /> There are several conceivable situations that could result in the loss of telephone <br /> communications from the mine. They include: <br /> (a) The incoming telephone line being served. <br /> (b) Major Snow storms. <br /> (c) Rangeland fires. <br /> (d) Power failure. <br /> (e) Wind Storms <br /> (f) Lighting Storms <br /> 5.2.4 Communications may therefore fail when they are most needed, i.e. in an emergency situation. <br /> Even if this is not the case, efficient operation of the mine is severely compromised without <br /> communications and a delay in notifying the relevant persons/agencies (Electrical Supervisor, IT <br /> Department, etc.)further exacerbates the problem. <br /> 5.2.5 Alternate Communication Plan: <br /> (a) Cell Phone <br /> (b) Radio communication with Teller County Sheriffs Office, Dispatch, Cripple Creek Fire Dept. <br /> and Cripple Creek Ambulance. <br /> I1 <br /> JII <br /> 5.3.1 Provide the same information as outlined under, Reporting an Emergency <br /> 5.3.2 Employee: Notifies his or her supervisor. <br /> 5.3.3 Supervisor: <br /> (a) Notifies security, his or her immediate supervisor, the area manager, and health and safety. <br /> (b) The immediate supervisor will limit available information to the fact of the accident, having <br /> made positive identification of any injured employee. <br /> (c) Radio discussions of the accident will be minimized and names and equipment numbers will <br /> not be broadcast. <br /> 5.3.4 The immediate supervisor will preserve the accident scene and all physical evidence until <br /> relieved of this responsibility. <br /> 5.3.5 Accident site visits will be limited to only necessary emergency response personnel, security <br /> personnel and appropriate management. <br /> 5.3.6 General Manager, Manager, Superintendent, General Foreman, reports via chain of command to <br /> implement Rapid Response and SRT Leaders. <br /> 5.3.7 Security: Initiates the following dispatches for the appropriate response. <br /> (a) Medical: Company medical responders or Teller County EMS. <br /> (b) Fire: Company MRT. or Cripple Creek Fire Department. <br /> (c) Rescue/Hazmat: MRT <br /> (d) Mutual Aid: Combined MRT or Teller County Cripple Creek <br /> (e) Area Health and Safety Manager or on call HSLP representative. <br /> 5.3.8 Cripple Creek&Victor-Superintendent, General Foreman, Manager, General Manager reports via <br /> chain of command to implement the Rapid Response and SRT Leaders. <br /> Department THIS DOCUMENT IS UNCONTROLLED IN HARD COPY FORMAT Doc Id:NA-CCV-HSLP-PLAN-1775 <br /> Health Safety&Loss Prevention Date of This Issue:25 June 2018 Date of Next Review:25 June 2019 Page 7 of 38 <br />
