Stormwater Management Plan <br />or discharge entering waters of the United States will be properly reported. The General <br />Contractor will prepare a written record of any spill and associated clean-up activities of <br />petroleum products or hazardous materials in excess of 1 gallon or reportable quantities, <br />which ever is less. The General Contractor will provide notice to George M.L. Robinson <br />or the Storm Water Administrator immediately upon identification of a reportable spill. <br />A spill report form is located in Appendix B. <br />Any spills of petroleum products or hazardous materials in excess of Reportable <br />Quantities as defined by EPA or the state or local agency regulations, shall be <br />immediately reported, in consultation with the Storm Water Administrator, to the EPA <br />National Response Center (1-800-424-8802), the Colorado Department of Public Health <br />and Environment (877-518-5608) and the Division of Reclamation, Mining and Safety <br />(303-866-3567). <br />3. BMPs Exhibit U (Attachment U-3, Figure U-1) will be implemented to manage stormwater and <br />to prevent sediment from entering the waters of the State at the following locations: <br />a. Portal benches, adits and the Base of Operation <br />1. Base of Operation -Exhibit D, Figure D-3 <br />2. May Day No. 1; (Exhibit D, Figure D-6) <br />3. May Day No. 1 east; <br />4. May Day No. 2; (Exhibit D, Figure D-7 and D-8)_ <br />5. Chief Portal (Manway)-;(Exhibit D, Figure D-10) <br />6. Idaho historical mine bench(Exhibit D, Figure D-11 and <br />b. Rock disposal and snow storage areas; <br />c. Little Deadwood Gulch drainage channel adjacent to the Chief Manway*; <br />d. Appropriate locations along mine access roads; and <br />e. Chemical and fuel storage locations. <br />4. When appropriate, the following stormwater management controls will be adopted: <br />a. Berms; <br />b. Check Dams; <br />c. Riprap/Gabions; <br />d. Sediment Traps/Debris Basins; <br />e. Sediment Basins and Rock Dams; <br />f. Silt Fences; and <br />g. Fiber Rolls (Wattles). <br />Inspection and Maintenance <br />a. Inspection Schedules <br />a.
