Stormwater Management Plan <br />erosion. The Construction Activity and Site Stabilization Log included in Appendix C <br />shall be updated following each major construction activity or monthly. <br />b. During snowy months (November through April), BMPs will be inspected and <br />maintained as field conditions dictate. The second year after BMPs are in place and <br />following the first winter event, BMPs will be inspected quarterly until the first snow fall <br />returns. <br />c. Inspection Procedures <br />• Verify and maintain the integrity of silt fences (USEPA, 1992), gravity walls, erosion <br />ditches, bar ditches, swales, check dams (US EPA, 1992)(VDCR, 2005), debris <br />basins and impoundments; <br />• Identify and repair rills and gullies that are greater than 12 inches deep; and <br />• maintain maintenance records. <br />d. BMP Maintenance/Replacement and Failed BMPs <br />Assuming access is achievable, BMP maintenance will be completed within 7 calendar days after <br />the identification of need for improvement or replacement. <br />e. Record Keeping and Documenting Inspections <br />Inspections and maintenance activities (Appendix D) will be documented and records will be kept <br />in the office file. <br />f. Agency Storm Water Inspections <br />A project representative must walk the site with the regulatory inspector and document any <br />deficiencies noted during the inspection. The company representative shall prepare an Inspection <br />Report for the day of the agency inspection. Deficiencies of any type, field or documentation - <br />related, identified during the regulatory inspection must be noted on the Inspection Report as a <br />deficiency and resolved as soon as practicable or as directed. <br />The representative must call the Storm Water Administrator to report the agency inspection <br />immediately, but no later than 1 -hour after the inspector has left the jobsite. All storm water or <br />erosion and sediment (E&S) agency visits to the jobsite, whether an official inspection occurred <br />or not, must be reported the Storm Water Administrator. Any agency inspector, including OSHA <br />and utility inspectors that comment on storm water BMPs (inlet protection, track out, erosion <br />control structure maintenance, etc.) must be reported to the Storm Water Administrator. <br />A log of all inspections by Federal, State, or local storm water or other environmental agencies <br />shall be kept in the SWMP Binder. The log form can be found in Appendix E and must include <br />the date and time of the visit and whether a report was issued or will be issued as a result of the <br />inspection. All inspection reports issued by an agency must be faxed or e-mailed to the Storm <br />Water Administrator immediately, but no later 24 -hours of receipt. <br />Sunrise Mining, LLC <br />Revised August 9, 2017 <br />Page 12 of 30 <br />
